乳腺浸润性导管癌上皮剪接调控蛋白1与赖氨酸去甲基化酶5A表达及临床意义

摘要 目的 探讨乳腺浸润性导管癌及癌旁组织中上皮剪接调控蛋白1(ESRP1)与赖氨酸去甲基化酶5A(KDM5A)的表达情况及临床意义。方法 采用回顾性研究方法,选取包头医学院第一附属医院2022-04-01-2023-04-30收治的手术治疗且经病理学检查确诊为乳腺浸润性导管癌的组织标本80例作为观察组,并以癌旁组织标本(距离癌组织>5 cm)作为对照组。经免疫组化法检测组织标本中ESRP1与KDM5A蛋白的表达情况。分析ESRP1与KDM5A乳腺浸润性导管癌及癌旁组织中的表达及与临床特征的关系。Spearman相关性分析两者相关性。结果 ESRP1和KDM5A在细胞核及细胞质中均有表达。ESRP1在乳腺浸润性导管癌中阳性表达率为56.25%(45/80),在癌旁组织中阳性表达率为15.00%(12/80),差异有统计学意义,χ^(2)=29.678,P<0.001。KDM5A在乳腺浸润性导管癌中的阳性表达率为28.75%(23/80),在癌旁组织中的阳性表达率为45.00%(36/80),差异有统计学意义,χ^(2)=4.538,P=0.033。乳腺浸润性导管癌组织中ESRP1蛋白表达与临床分期有关(χ^(2)=12.989,P<0.001),KDM5A蛋白表达与淋巴结转移(χ^(2)=9.104,P=0.003)、神经侵犯(χ^(2)=9.426,P=0.002)和临床分期(χ^(2)=7.999,P=0.005)有关。乳腺癌浸润性导管癌组织中ESRP1与KDM5A蛋白表达呈负相关,r=-0.335,P<0.001。结论 ESRP1和KDM5A可能作为乳腺浸润性导管癌的潜在生物标志物,其表达水平与乳腺癌的临床特征密切相关,为乳腺癌的早期诊断、侵袭性评估及个体化治疗提供了新的分子靶点和理论依据。 Objective To investigate the expression and clinical significance of epithelial splicing regulatory protein 1(ESRP1)and lysine demethylase 5A(KDM5A)in breast invasive ductal carcinoma and adjacent tissues.Methods A retrospective study was conducted using 80cases of breast invasive ductal carcinoma tissue specimens diagnosed by pathological examination and treated surgically at The First Affiliated Hospital of Baotou Medical College from April 1,2022,to April 30,2023,as the observation group.Adjacent tissue specimens(>5cm from the tumor)were used as the control group.The expression of ESRP1and KDM5Aproteins in the tissue specimens was detected by immunohistochemistry.The relationship between the expression of ESRP1and KDM5Ain breast invasive ductal carcinoma and adjacent tissues and clinical characteristics was analyzed.Spearman correlation analysis was used to assess the correlation between the two proteins.Results ESRP1and KDM5Awere expressed in both the nucleus and cytoplasm.The positive expression rate of ESRP1in breast invasive ductal carcinoma was 56.25%(45/80),significantly higher than that in adjacent tissues(15.00%,12/80),with a statistically significant difference(χ^(2)=29.678,P<0.001).The positive expression rate of KDM5Ain breast invasive ductal carcinoma was 28.75%(23/80),significantly lower than that in adjacent tissues(45.00%,36/80),with a statistically significant difference(χ^(2)=4.538,P=0.033).The expression of ESRP1in breast invasive ductal carcinoma was related to clinical stage(χ^(2)=12.989,P<0.001),while the expression of KDM5Awas related to lymph node metastasis(χ^(2)=9.104,P=0.003),neural invasion(χ^(2)=9.426,P=0.002),and clinical stage(χ^(2)=7.999,P=0.005).The expression of ESRP1and KDM5Ain breast invasive ductal carcinoma showed a negative correlation(r=-0.335,P<0.001).Conclusions ESRP1and KDM5Amay serve as potential biomarkers for breast invasive ductal carcinoma.Their expression levels are closely related to the clinical characteristics of breast cancer,providing new molecular targets and theoretical basis for early diagnosis,invasiveness assessment,and individualized treatment of breast cancer.
