摘要
目的 观察盐酸二甲双胍缓释片联合慢性病管理对2型糖尿病患者血糖控制效果和健康行为的影响。方法 83例住院治疗的2型糖尿病患者,以随机数字表法分为对照组(41例)与研究组(42例)。对照组接受盐酸二甲双胍缓释片治疗联合常规管理,研究组在对照组基础上接受慢性病管理。对比两组患者入院时及出院后3个月的健康行为评分、血糖指标、糖尿病生存质量特异性量表(DSQL)评分,血糖控制效果。结果 出院后3个月,两组患者遵医用药、适量运动、合理饮食、自我血糖监测和足部护理评分均较本组入院时升高,且研究组患者遵医用药评分(6.07±1.12)分、适量运动评分(5.14±0.98)分、合理饮食评分(5.86±1.03)分、自我血糖监测评分(4.98±0.85)分、足部护理评分(4.92±0.75)分较对照组的(5.14±0.93)、(4.28±0.91)、(4.59±0.97)、(3.74±0.77)、(4.03±0.63)分高(P<0.05)。出院后3个月,两组患者空腹血糖、餐后2 h血糖、糖化血红蛋白水平均较本组入院时下降,且研究组患者空腹血糖(6.29±0.92)mmol/L、餐后2 h血糖(9.29±1.75)mmol/L、糖化血红蛋白(6.02±1.04)%较对照组的(7.18±1.25)mmol/L、(10.53±2.11)mmol/L、(7.11±1.26)%低(P<0.05)。出院后3个月,两组患者治疗、社会关系、心理、生理评分均较本组入院时下降,且研究组患者治疗评分(3.55±0.52)分、社会关系评分(4.19±0.85)分、心理评分(14.27±2.55)分、生理评分(22.03±2.95)分较对照组的(4.78±0.81)、(5.24±0.93)、(17.03±2.87)、(26.19±3.65)分低(P<0.05)。研究组患者血糖控制达标率92.86%(39/42)较对照组的73.17%(30/41)高(P<0.05)。结论 盐酸二甲双胍缓释片联合慢性病管理有利于提高2型糖尿病患者的血糖控制效果,促进健康行为的形成,改善患者生活质量,值得推广。
Objective To observe the effect of metformin hydrochloride sustained-release tablets combined with chronic disease management on blood glucose control and health behavior of patients with type 2 diabetes mellitus.Methods 83 inpatients with type 2 diabetes mellitus were divided into a control group(41 cases)and a research group(42 cases)by random number table method.The control group was treated with metformin hydrochloride sustained-release tablets and routine management,and the research group was given chronic disease management on the basis of the control group.The health behavior score,blood glucose index and diabetes specific quality of life scale(DSQL)score at 3 months after discharge and blood glucose control effect of the two groups of the two groups were compared.Results At 3 months after discharge,At 3 months after discharge,the scores of medication compliance,moderate exercise,reasonable diet,self-blood glucose monitoring and foot care in both groups were higher than those at admission in this group;the medication compliance score in the research group was(6.07±1.12)points,the moderate exercise score was(5.14±0.98)points,the reasonable diet score was(5.86±1.03)points,the self-blood glucose monitoring score was(4.98±0.85)points,and the foot care score was(4.92±0.75)points,which were higher than(5.14±0.93),(4.28±0.91),(4.59±0.97),(3.74±0.77),and(4.03±0.63)points in the control group(P<0.05).At 3 months after discharge,the levels of fasting blood glucose,2-h postprandial blood glucose and glycated hemoglobin in both groups decreased compared with at admission in this group;in the research group,the fasting blood glucose was(6.29±0.92)mmol/L,the 2-h postprandial blood glucose was(9.29±1.75)mmol/L,and the glycated hemoglobin was(6.02±1.04)%,which were lower than(7.18±1.25)mmol/L,(10.53±2.11)mmol/L,and(7.11±1.26)%in the control group(P<0.05).At 3 months after discharge,the scores of treatment,social relationship,psychology and physiology in both groups decreased compared with at admission in this group;in the research group,the treatment score was(3.55±0.52)points,the social relationship score was(4.19±0.85)points,the psychology score was(14.27±2.55)points,and the physiology score was(22.03±2.95)points,which were lower than(4.78±0.81),(5.24±0.93),(17.03±2.87),and(26.19±3.65)points in the control group(P<0.05).The standard-achieving rate of blood glucose control in the research group was 92.86%(39/42),which was higher than 73.17%(30/41)in the control group(P<0.05).Conclusion Metformin hydrochloride sustained-release tablets combined with chronic disease management is beneficial to improving the blood glucose control of patients with type 2 diabetes mellitus,promoting the formation of healthy behaviors,and improving the quality of life,which is worth popularizing.
