摘要
目的 探讨CT联合磁共振成像(MRI)诊断非典型肾上腺节细胞神经瘤的价值及影像学表现。方法 40例疑似非典型肾上腺节细胞神经瘤患者,均行CT、MRI检查。以临床病理学检查结果作为金标准,比较CT检查、MRI检查、联合检查的检出率及诊断效能,分析CT检查、 MRI检查、病理学检查的结果。结果 临床病理学检查(金标准)结果显示,阳性39例,阴性1例。CT检查显示阳性30例,阴性10例;MRI检查显示阳性32例,阴性8例;联合检查显示阳性38例,阴性2例。联合检查的检出率95.00%高于CT检查的75.00%及MRI检查的80.00%,存在差异(P<0.05);CT检查与MRI检查的检出率比较,无统计学意义(P>0.05)。CT检查与MRI检查的灵敏度、特异度、准确度比较,无统计学意义(P>0.05);联合检查的灵敏度97.44%、准确度97.50%高于CT检查的74.36%、72.50%及MRI检查的79.49%、77.50%,存在差异(P<0.05);联合检查的特异度与CT检查、MRI检查比较,无统计学意义(P>0.05)。结论 CT联合MRI诊断非典型肾上腺节细胞神经瘤的灵敏度、准确度均高于CT、MRI单独诊断,值得临床推广应用。
Objective To explore the value and imaging findings of CT combined with magnetic resonance imaging(MRI)in the diagnosis of atypical adrenal ganglioneuroma.Methods 40 patients with suspected atypical adrenal ganglioneuroma all underwent CT and MRI examinations.Taking the results of clinicopathological examination as the gold standard,the detection rates and diagnostic efficacy of CT examination,MRI examination and combined examination were compared,and the results of CT examination,MRI examination and pathological examination were analyzed.Results Clinicopathologic examination(gold standard)showed 39 positive cases and 1 negative case;CT examination showed 30 positive cases and 10 negative cases;MRI examination showed 32 positive cases and 8 negative cases;combined examination showed 38 positive cases and 2 negative cases.The detection rate of combined examination was 95.00%,which was higher than 75.00%of CT examination and 80.00%of MRI examination,and there was a difference(P<0.05).Comparison of the detection rates between CT examination and MRI examination showed no statistical significance(P>0.05).There was no statistical difference in the sensitivity,specificity and accuracy between CT examination and MRI examination(P>0.05).The sensitivity of combined examination was 97.44%and the accuracy was 97.50%,which were higher than 74.36%and 72.50%of CT examination,and 79.49%and 77.50%of MRI examination,and there were differences(P<0.05).The specificity of the combined examination was not statistically significant compared with that of CT examination and MRI examination(P>0.05).Conclusion The sensitivity and accuracy of CT combined with MRI in the diagnosis of atypical adrenal ganglioneuroma are both higher than those of CT and MRI alone,and it is worthy of clinical promotion and application.
