限制-诱导运动疗法在痉挛型偏瘫患儿康复中的应用效果分析

摘要 目的 分析痉挛型偏瘫患儿应用限制-诱导运动疗法的康复效果。方法 选择痉挛型偏瘫患儿100例,按康复方法的不同分为常规组、研究组,各50例。常规组采用常规康复干预,研究组在常规组基础上采用限制-诱导运动疗法干预。对比两组非偏瘫侧和偏瘫运动发育评分、手功能评分。结果 干预前及干预30、90 d,两组非偏瘫侧粗大运动商、精细运动商、总运动商评分对比差异性并不明显(P>0.05)。干预前,两组偏瘫侧粗大运动商、精细运动商、总运动商评分对比差异性并不明显(P>0.05);干预30和90 d,研究组偏瘫侧粗大运动商、精细运动商、总运动商评分分别为(82.81±7.82)、(79.14±5.36)、(79.58±5.45)分和(86.54±7.46)、(83.34±4.97)、(85.35±6.96)分,均高于常规组的(76.52±7.76)、(74.76±5.46)、(75.56±4.61)分和(80.45±7.67)、(79.86±4.55)、(80.06±7.03)分,存在统计学意义(P<0.05)。干预前及干预30、90 d,两组非偏瘫侧手功能评分对比差异性并不明显(P>0.05)。干预前,两组偏瘫侧手功能评分对比差异性并不明显(P>0.05);干预30和90 d,研究组手功能评分分别为(56.44±4.85)、(63.14±6.68)分,均高于常规组的(53.16±4.43)、(58.16±5.75)分,存在统计学意义(P<0.05)。结论 在痉挛型偏瘫患儿康复过程中使用限制-诱导运动疗法干预,能够有效改善运动发育情况和手功能水平,且不会对非偏瘫侧肢体运动造成影响,临床可广泛应用。 Objective To analyze the effect of constraint-induced movement therapy in rehabilitation of children with spastic hemiplegia.Methods 100 children with spastic hemiplegia were selected and divided into a conventional group and a study group according to different rehabilitation methods,with 50 cases in each group.The conventional group received conventional rehabilitation,while the study group received constraint-induced movement therapy on the basis of the conventional group.The motor development scores and hand function scores of the non hemiplegic and hemiplegic sides were compared between the two groups.Results Before the intervention and 30 and 90 d after intervention,there was no significant difference in the scores of gross motor quotient,fine motor quotient and total motor quotient on the non-hemiplegic side between the two groups(P>0.05).Before intervention,there was no significant difference in the scores of gross motor quotient,fine motor quotient and total motor quotient on the hemiplegic side between the two groups(P>0.05).30 and 90 d after intervention,the scores of gross motor quotient,fine motor quotient and total motor quotient on the hemiplegic side of the study group were(82.81±7.82),(79.14±5.36),(79.58±5.45)points and(86.54±7.46),(83.34±4.97),(85.35±6.96)points,which were higher than(76.52±7.76),(74.76±5.46),(75.56±4.61)points and(80.45±7.67),(79.86±4.55),(80.06±7.03)points of the conventional group,and there were statistical significance(P<0.05).Before the intervention and 30 and 90 d after intervention,there was no significant difference in hand function scores on the non-hemiplegic side between the two groups(P>0.05).Before intervention,there was no significant difference in hand function scores on the hemiplegic side between the two groups(P>0.05).30 and 90 d after intervention,the hand function scores of the study group were(56.44±4.85)and(63.14±6.68)points,which were higher than(53.16±4.43)and(58.16±5.75)points of the conventional group,and there were statistical significance(P<0.05).Conclusion Constraint-induced movement therapy in rehabilitation of children with spastic hemiplegia can effectively improve the motor development and hand function level,and will not affect the limb movement on the non-hemiplegic side.It can be widely used in clinical practice.
