摘要
目的探讨稳心颗粒联合美托洛尔在冠心病合并室性心律失常患者中的应用效果。方法94例冠心病合并室性心律失常患者,利用整群抽样随机划分为对照组和观察组,各47例。两组均采用常规方法治疗,在此基础上,对照组采用美托洛尔治疗,观察组采用美托洛尔联合稳心颗粒治疗。比较两组的中医证候积分、血液流变学指标、血管内皮功能指标[血清一氧化氮(NO)、血管性血友病因子(vWF)及内皮素-1(ET-1)]及不良反应发生情况。结果治疗后,两组心慌、胸闷、气短、畏寒肢冷、神倦体乏及头晕积分均较本组治疗前降低,且观察组心慌积分(1.12±0.34)分、胸闷积分(0.96±0.31)分、气短积分(0.99±0.25)分、畏寒肢冷积分(1.21±0.26)分、神倦体乏积分(1.35±0.32)分及头晕积分(1.29±0.28)分较对照组的(2.96±0.43)、(2.65±0.47)、(2.72±0.32)、(2.42±0.34)、(2.16±0.45)、(2.42±0.37)分低(P<0.05)。治疗后,两组纤维蛋白原、全血低切粘度、全血高切粘度及血浆比粘度均较本组治疗前降低,且观察组纤维蛋白原(2.35±0.31)g/L、全血低切粘度(5.35±0.96)mPa·s、全血高切粘度(3.21±0.85)mPa·s及血浆比粘度(1.41±0.38)mPa·s较对照组的(3.01±0.42)g/L、(6.61±1.15)mPa·s、(4.69±0.92)mPa·s、(1.88±0.41)mPa·s低(P<0.05)。治疗后,两组NO水平较本组治疗前高,ET-1及vWF水平较本组治疗前低,且观察组NO水平较对照组高,ET-1及vWF水平较对照组低(P<0.05)。两组不良反应发生率比较无统计差异(P>0.05)。结论稳心颗粒联合美托洛尔能降低冠心病合并室性心律失常患者的中医证候积分,改善血液流变学及血管内皮功能,且药物安全性较高,值得临床推广应用。
Objective To explore the application effect of Wenxin Granule combined with metoprolol in the treatment of patients with coronary heart disease complicated with ventricular arrhythmia.Methods 94 patients with coronary heart disease complicated with ventricular arrhythmia were randomly divided into a control group and an observation group by cluster sampling,with 47 cases in each group.Both groups were treated with conventional methods.On this basis,the control group was treated with metoprolol,and the observation group was treated with metoprolol in combination with Wenxin Granule.The traditional Chinese medicine syndrome score,hemorheology indexes,vascular endothelial function indexes[serum nitric oxide(NO),von Willebrand factor(vWF),and endothelin-1(ET-1)]and occurrence of adverse reactions of the two groups were compared.Results After the treatment,the scores of palpitations,chest tightness,shortness of breath,chills and cold limbs,listlessness and physical weakness,and dizziness in both groups were lower than those before the treatment in this group;in the observation group,the score of palpitations was(1.12±0.34)points,the score of chest tightness was(0.96±0.31)points,the score of shortness of breath was(0.99±0.25)points,the score of chills and cold limbs was(1.21±0.26)points,the score of listlessness and physical weakness was(1.35±0.32)points,and the score of dizziness was(1.29±0.28)points,which were lower than(2.96±0.43),(2.65±0.47),(2.72±0.32),(2.42±0.34),(2.16±0.45),and(2.42±0.37)points in the control group(P<0.05).After the treatment,the fibrinogen,whole blood low shear viscosity,whole blood high shear viscosity and plasma viscosity in both groups were decreased compared with before the treatment in this group;in the observation group,the fibrinogen was(2.35±0.31)g/L,the whole blood low shear viscosity was(5.35±0.96)mPa·s,the whole blood high shear viscosity was(3.21±0.85)mPa·s,and the plasma viscosity(1.41±0.38)mPa·s,which were lower than(3.01±0.42)g/L,(6.61±1.15)mPa·s,(4.69±0.92)mPa·s,and(1.88±0.41)mPa·s in the control group(P<0.05).After the treatment,the levels of NO in both groups were higher than those before the treatment in this group,while the levels of ET-1 and vWF were lower than those before the treatment in this group;the level of NO in the observation group was higher than that in the control group,while the levels of ET-1 and vWF were lower than those in the control group(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Wenxin Granule combined with metoprolol can reduce the traditional Chinese medicine syndrome score of patients with coronary heart disease complicated with ventricular arrhythmia,improve hemorheology and vascular endothelial function.It has high safety,and is worthy of promotion and application.
