首发精神分裂症灰质结构影像学的研究进展

摘要 精神分裂症是一种严重的精神疾病,也是全球疾病负担和致残的主要原因之一。近年来影像研究发现,首发精神分裂症(FES)存在脑结构异常,但各研究报道的异常区域存在一定的异质性。纵向研究显示,FES的脑结构可能受到药物和病程影响。本文综述近年来FES结构影像学研究进展,发现FES患者存在灰质结构异常,尤其是灰质体积减小,且这种变化可能并非局限于单一脑区,而是涉及脑网络层面的广泛改变。FES的结构异常呈现进行性变化,可能与药物治疗相关。目前关于FES与其他精神疾病的比较研究较少,未来需进一步探索FES的结构网络变化及其与其他精神疾病的异同,以期为疾病的精准诊断和治疗提供更多理论依据及方向。 Schizophrenia is a severe mental disease,and is also one of the main causes of global disease burden and disability.In recent years,imaging studies have found that first episode schizophrenia(FES)has brain structural abnormalities,but there is some heterogeneity in the abnormal areas reported in various studies.Longitudinal studies have shown that brain structure of FES may be influenced by drugs and disease progression.This article reviews recent progress in structural imaging research of FES and finds that patients with FES have abnormal gray matter structures,especially reduced gray matter volume,these changes may not be limited to a single brain region,but involve extensive changes at the level of brain networks.Structural abnormalities of FES exhibit progressive changes,which may be related to drug therapy.At present,there is relatively little comparative research on FES and other mental illnesses,in the future,further exploration is needed on structural network changes of FES,and its similarities and differences with other mental illnesses,in order to provide more theoretical basis and direction for accurate diagnosis and treatment of diseases.
