摘要
目的 探讨靶控输注异丙酚联合利多卡因全静脉麻醉的临床价值。方法 选取2023年1—12月临沂市精神卫生中心收治的行全静脉麻醉的手术治疗患者100例,随机分为对照组与观察组,各50例。对照组使用靶控输注异丙酚进行全静脉麻醉处理,观察组采用靶控输注异丙酚联合利多卡因全静脉麻醉处理。比较两组患者血压、心率、麻醉效果、认知功能、疼痛状况、麻醉优良率。结果 麻醉前两组收缩压、舒张压、心率差异不显著(P>0.05);麻醉后观察组收缩压、舒张压、心率低于对照组(P<0.05)。观察组术后苏醒时间、自主呼吸恢复时间、术后拔管时间短于对照组(P<0.05)。两组麻醉前简易智力状态检查量表(MMSE)、视觉模拟评分法(VAS)评分比较差异不显著(P>0.05);麻醉后观察组MMSE、VAS评分均优于对照组(P<0.05)。观察组麻醉优良率高于对照组,差异有统计学意义(P<0.05)。结论 为手术患者选取靶控输注异丙酚联合利多卡因进行全静脉麻醉的效果较好,可稳定血压与心率波动,减轻疼痛,加快术后苏醒,缩短拔管时间,改善患者认知功能。
Objective To explore the clinical value of total intravenous anesthesia with target-controlled infusion of propofol combined with lidocaine.Methods A total of 100 patients who underwent surgical treatment under total intravenous anesthesia and were admitted to Linyi Mental Health Center from January to December 2023 were selected and randomly divided into a control group and an observation group,with 50 cases in each group.The control group was treated with total intravenous anesthesia by target-controlled infusion of propofol,while the observation group was treated with total intravenous anesthesia by target-controlled infusion of propofol combined with lidocaine.The blood pressure,heart rate,anesthesia effect,cognitive function,pain status and excellent and good rate of anesthesia of the two groups of patients were compared.Results Before anesthesia,there was no significant difference in systolic blood pressure,diastolic blood pressure and heart rate between the two groups(P>0.05).After anesthesia,the systolic blood pressure,diastolic blood pressure and heart rate in the observation group were lower than those in the control group(P<0.05).The postoperative awakening time,recovery time of spontaneous breathing and postoperative extubation time in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in the scores of the Mini-Mental State Examination(MMSE)and Visual Analogue Scale(VAS)before anesthesia between the two groups(P>0.05).After anesthesia,the MMSE and VAS scores of the observation group were both better than those of the control group(P<0.05).The excellent and good rate of anesthesia in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The effect of selecting target-controlled infusion of propofol combined with lidocaine for total intravenous anesthesia for surgical patients is better.It can reduce blood pressure and heart rate fluctuations,relieve pain,accelerate postoperative recovery,shorten extubation time,and improve the cognitive function of patients.
