摘要
目的 构建长沙地区科学合理的院前急救服务能力评价指标体系,为持续提高院前急救服务能力质量提供参考依据。方法 在深入访谈、实地调研、文献研究的基础上,进行两轮德尔菲法专家询函,运用层次分析法确定每个指标的权重。结果 两轮专家咨询的应答率均为100%,专家权威系数0.88,协调系数分别为0.336和0.377(P<0.001),最终形成涵盖4个一级指标、9个二级指标和36个三级指标的长沙地区院前急救服务能力评价指标体系。各指标重要性评分均值均大于3.5分,变异系数均小于0.33。一级指标包括站点建设、调度管理、医疗质量、服务质量,对应权重为0.22、0.21、0.48、0.09;在二级指标权重值中,医疗处置(0.17)、调度质量(0.12)、人员配备(0.12)、病历书写(0.11)位列前四。结论 通过德尔菲法构建的院前急救服务能力评价指标体系,提升院前急救服务的质量和效率,提高院前急危重症患者生存质量。
Objective Establish a scientifically sound evaluation index system for pre-hospital emergency care service capacity in the Changsha region,providing a reference framework for the con-tinuous improvement of pre-hospital emergency care quality.MethodsBased on in-depth interviews,field research,and literature review,two rounds of Delphi expert consultations were conducted,and the Analytic Hierarchy Process(AHP)was applied to determine the weight of each indicator.Results The response rate of the two rounds of expert consultation was 100%,the expert authority coeffi-cient was 0.88,and the coordination were 0.336 and 0.377(P value<0.001),respectively.Final-ly,an evaluation index system the pre-hospital emergency service capacity in Changsha area was formed,covering 4 primaryindicators,9 secondary indicators and 36 tertiaryindicators.The mean importance score of each indicator was greater than 3.5 points,and the coefficient of variation was less than 0.33.The first-level indicators included station construction,dispatchmanagement,medical quality,and service quality,with corresponding weights of 0.22,0.21,0.48,and 0.09;among the secondary indicator weight values,medical treatment(0.17),dispatch quality(0.12),personnel al-location(0.12),and medical record writing(0.11)ranked in the top four.Conclusions The pre-hospital emergency service capacity evaluation index system constructed by the Delphi method,to im-prove the quality and efficiency of pre-hospital emergency services,to improve the survival quality of patients with critical illness in pre-hospital emergency.
