摘要
目的探讨超声引导下针刀松解腘肌联合加味紫金丹外敷治疗腘肌急慢性损伤的临床效果。方法收集2021年4月至2022年4月收治的腘肌急慢性损伤患者60例,随机分为观察组和对照组,各30例。对照组采用传统的针刀松解腘肌治疗,观察组采用超声引导下针刀松解腘肌联合加味紫金丹外敷治疗。两组术后都进行膝关节自主功能训练,比较两组临床疗效、疼痛视觉模拟评分法(VAS)评分、膝关节功能活动度、炎症指标、不良反应。结果治疗后,两组在不同时间点VAS评分呈下降趋势,且观察组低于对照组,差异有统计学意义(P<0.05)。治疗后即刻与治疗后2周,两组的伸、屈膝关节最大角分别与治疗前相比较均有改善,且观察组活动范围优于对照组,差异有统计学意义(P<0.05)。治疗后,两组炎症指标均较治疗前下降,且治疗后1周观察组低于对照组,差异有统计学意义(P<0.05)。治疗后2周观察组治疗总有效率为86.67%,较对照组的46.67%高,差异有统计学意义(P<0.05)。两组均无不良事件发生。结论采用超声引导下针刀松解腘肌联合加味紫金丹外敷治疗腘肌急慢性损伤患者,可提高临床疗效,减轻疼痛症状,增加关节活动度,降低炎症指标。
Objective To explore the clinical effect of ultrasound-guided needle knife release of the popliteus muscle combined with modified Zijin Dan external application in the treatment of acute and chronic popliteus muscle injury.Methods Sixty patients with acute and chronic popliteus muscle injury admitted from April 2021 to April 2022 were randomly divided into an observation group and a control group,with 30 cases in each group.The control group was treated with traditional needle knife release of popliteus muscle,while the observation group was treated with ultrasound-guided needle knife release of popliteus muscle combined with modified Zijin Dan external application.Both groups underwent knee joint autonomous function training after surgery.Clinical efficacy,pain visual analog scale(VAS)scores,knee joint functional range of motion,inflammatory indicators and adverse reactions were compared between the two groups.Results After treatment,the VAS scores of the two groups showed a decreasing trend at different time points,and the observation group was lower than the control group,with a statistically significant difference(P<0.05).Immediately after treatment and 2 weeks after treatment,the maximum angles of extension and flexion joints in both groups were improved compared to before treatment,and the range of motion in the observation group was better than that in the control group,with a statistically significant difference(P<0.05).After treatment,both groups showed a decrease in inflammatory indicators compared to before treatment,the observation group was lower than the control group one week after treatment,with a statistically significant difference(P<0.05).After 2 weeks of treatment,the total effective rate of the observation group was 86.67%,which was higher than that of the control group 46.67%,and the difference was statistically significant(P<0.05).No adverse events occurred in either group.Conclusion The use of ultrasound-guided needle knife release of popliteus muscle combined with modified Zijin Dan external application can improve clinical efficacy,alleviate pain symptoms,increase joint motion and reduce inflammatory indicators in patients with acute and chronic popliteus muscle injury.
