摘要
目的 了解离退休老年共病患者心理困扰的现状与影响因素,为实施降低患者的心理困扰水平的干预措施提供依据。方法 采取便利抽样的方法选取2023年1月至12月在湖北省武汉市某三级甲等医院老年病科住院的245例离退休老年共病患者为研究对象,采用问卷调查法收集患者的一般资料、疾病累积评分、护理依赖水平、心理困扰水平,疾病感知水平及应对方式,应用Logistic回归分析探索离退休老年共病患者心理困扰的影响因素。结果 共纳入245例患者,轻度、中度、重度和完全护理依赖患者的占比分别为54.3%、21.2%、19.2%、5.3%。离退休老年共病患者心理困扰发生率为70.6%。Logistic回归分析显示,中度、重度及完全护理依赖(OR值分别为2.656,7.389,41.888)、疾病感知水平(OR值1.177)与消极应对方式(OR值16.007)对离退休老年共病患者心理困扰具有正向预测作用,积极应对方式(OR值0.153)具有负向预测作用。结论 离退休老年共病患者心理困扰发生率高,中度、重度及完全护理依赖,疾病感知水平与消极应对方式是离退休老年共病患者心理困扰的独立危险因素;积极应对方式具有保护作用。应尽早识别危险因素,采取干预措施,帮助采取积极应对方式,维护患者心理健康。
Objective To understand the current status and influencing factors of psychological distress in retired elderly patients with multimorbidity,and provide evidence for interventions aimed at reducing psychological distress in these patients.Methods Using a convenience sampling method,we selected 245 retired elderly patients with multimorbidity who were hospitalized in the Department of Geriatrics at a tertiary hospital in Wuhan,Hubei,from January to December 2023.A questionnaire survey was used to collect general information,disease accumulation scores,care dependency levels,psychological distress levels,disease perception,and coping styles.Logistic regression analysis was applied to explore the factors influencing psychological distress in retired elderly patients with multimorbidity.Results A total of 245 patients were included.The proportions of patients with mild,moderate,severe,and complete care dependency were 54.3%,21.2%,19.2%,and 5.3%,respectively.The incidence of psychological distress in retired elderly patients with multimorbidity was 70.6%.Logistic regression analysis showed that moderate,severe,and complete care dependency(OR values of 2.656,7.389,and 41.888,respectively),disease perception level(OR value of 1.177),and negative coping styles(OR value of 16.007)were positively associated with psychological distress.In contrast,positive coping styles(OR value of 0.153)were negatively associated with psychological distress.Conclusion The incidence of psychological distress is high in retired elderly patients with multimorbidity.Moderate,severe,and complete care dependency,disease perception level,and negative coping styles are independent risk factors for psychological distress,while positive coping styles serve a protective role.Early identification of risk factors and intervention measures,including encouraging positive coping styles,are essential to maintain the psychological health of these patients.
