神经电生理技术在糖尿病周围神经病变中的诊断价值 被引量:1

Diagnostic value of neuroelectrophysiological techniques in diabetic peripheral neuropathy

摘要 目的探讨神经电生理技术诊断糖尿病周围神经病变(DPN)的价值。方法选择蚌埠市第三人民医院2020年1月至2024年5月收治的100例DPN患者纳入DPN组,选择同期收治的100例单纯糖尿病(DM)患者,纳入单纯DM组,性别不限,年龄35~77岁。所有患者采用肌电图诱发电位进行电生理检测,比较DPN组与单纯DM组患者各神经的运动传导速度(MCV)和感觉传导速度(SCV),统计两组患者MCV和SCV异常例数,同时观察上下肢神经传导异常情况,及轻、中、重度DPN组MCV和SCV。结果DPN组正中神经、尺神经、胫神经及腓总神经的MCV与SCV均低于单纯DM组(P<0.001)。轻、中、重度DPN组神经传导速度存在明显差异(P<0.001),各神经传导速度随病变程度的加重而降低。DPN组神经传导异常率高于单纯DM组,且DPN患者中SVC异常率高于MCV异常率,下肢神经传导异常率高于上肢(P均<0.05)。结论DPN患者神经传导速度明显低于单纯DM患者,神经电生理技术可以为DPN的诊断提供准确客观的参考数据。 Objective To investigate the value of neuroelectrophysiological techniques in the diagnosis of diabetic peripheral neuropathy(DPN).Methods One hundred DPN patients admitted to the Third People's Hospital of Bengbu from January 2020 to May 2024 were selected and included in the DPN group,and 100 patients with simple DM were selected in the same period to be included in the simple DM group,both of all sexes,35 to 77 years old.Electrophysiological testing was performed in all patients.The motor conduction velocity(MCV)and sensory conduction velocity(SCV)of each nerve in DPN group and DM group were compared,the number of abnormal MCV and SCV cases in the two groups were counted,and abnormal nerve conduction in upper and lower limbs,as well as MCV and SCV in mild,moderate and severe DPN groups were observed.Results The MCV and SCV of median nerve,ulnar nerve,tibial nerve and common peroneal nerve in DPN group were lower than those in DM group(P<0.001).There were significant differences in nerve conduction velocity among mild,moderate and severe DPN groups(P<0.001),and the nerve conduction velocity decreased with the severity of the disease.The abnormal rate of nerve conduction in DPN group was higher than that in DM group,and the abnormal rate of SVC in DPN patients was higher than that of MCV,and the abnormal rate of nerve conduction in lower limbs was higher than that in upper limbs(all P<0.05).Conclusion The nerve conduction velocity of DPN patients is significantly lower than that of simple DM patients.Neuroelectrophysiological techniques can provide accurate and objective reference data for the diagnosis of DPN.
