DCG联合冠状动脉CT-FFR对冠心病心肌缺血的诊断价值

摘要 目的探讨动态心电图(DCG)联合冠状动脉CT血流储备分数(CT-FFR)诊断冠心病心肌缺血的价值。方法选取2024年12月至2025年2月我院收治的100例疑似冠心病心肌缺血患者作为研究对象,均进行DCG、冠状动脉CT-FFR、冠状动脉造影(CAG)检查。患者根据CAG结果分为阳性组(n=67)和阴性组(n=33),比较两组的一般资料。以CAG结果作为“金标准”,分析DCG、冠状动脉CT-FFR以及二者联合对冠心病心肌缺血的诊断效能。结果阳性组和阴性组的一般资料比较,差异无统计学意义(P>0.05)。以CAG检测结果作为“金标准”,DCG检出阳性63例,阴性37例;冠状动脉CT-FFR检出阳性63例,阴性37例;二者联合检出阳性66例,阴性34例。DCG联合冠状动脉CT-FFR诊断冠心病心肌缺血的敏感度、特异度、准确度、阴性预测值、阳性预测值分别为95.52%、93.94%、95.00%、91.18%、96.97%,均高于DCG、冠状动脉CT-FFR单独诊断;ROC曲线显示,DCG联合冠状动脉CT-FFR检测诊断冠心病心肌缺血的AUC值高于单独检测(Z=4.598,3.949,P<0.05)。结论DCG联合冠状动脉CT-FFR诊断冠心病心肌缺血的效能较高,有助于临床更好地对冠心病心肌缺血疾病进行诊断。 Objective To explore the value of dynamic electrocardiogram(DCG)combined with coronary CT angiography derived fractional flow reserve(CT-FFR)in the diagnosis of myocardial ischemia of coronary heart disease.Methods A total of 100 patients with suspected myocardial ischemia of coronary heart disease admitted to our hospital from December 2024 to February 2025 were selected as the research subjects,and all cases underwent DCG,coronary CT-FFR,and coronary angiography(CAG)examinations.Patients were divided into the positive group(n=67)and the negative group(n=33)based on CAG results,and the general information was compared between the two groups.Using CAG results as the"gold standard",the diagnostic efficacy of DCG,coronary artery CT-FFR,and their combination for myocardial ischemia of coronary heart disease was analyzed.Results The general information comparison between the positive group and the negative group showed no statistically significant difference(P>0.05).Using CAG test results as the"gold standard",DCG detected 63 positive cases and 37 negative cases;Coronary CT-FFR detected 63 positive cases and 37 negative cases;DCG combined with coronary CT-FFR detected 66 positive cases and 34 negative cases.The sensitivity,specificity,accuracy,negative predictive value,and positive predictive value of DCG combined with coronary CT-FFR in diagnosing myocardial is chemia of coronary heart disease were 95.52%,93.94%,95.00%,91.18%,and 96.97%,respectively,higher than those ofDCG and coronary CT-FFR alone;The ROC curve showed that the A UC value of DCG combined with coronary CT-FFR detection in diagnosing myocardial ischemia of coronary heart disease was higher than that of DCG and coronary CT-FFR alone(Z=4.598,3.949;P<0.05).Conclusions DCG combined with coronary CT-FFR in the diagnosis of myocardial ischemia of coronary heart disease has high efficacy,which is helpful for better clinical diagnosis of myocardial ischemia ofcoronary heart disease.
