摘要
目的 探讨^(18)F-氟代脱氧葡萄糖(FDG)正电子发射断层成像(PET)/MR多模态成像在肺部罕见恶性肿瘤中的影像诊断价值。方法 回顾性分析2019年6月至2024年2月间行^(18)F-FDG PET/MR的5例肺部罕见恶性肿瘤患者的临床、^(18)F-FDG PET/MR及病理资料,包括数量、部位、形态、大小、征象、信号、周围侵犯、全身转移、表观扩散系数(ADC)值及标准摄取值(SUV)等指标。结果 5例病灶DWI序列均为高信号,3例肺肉瘤样癌ADC值范围(0.82~2.15)×10^(-3) mm^(2)/s,ADC平均值1.38×10^(-3) mm^(2)/s,SUVmax范围3.96~35.23,SUVmax平均值10.36,2例肺腺鳞癌ADC值范围(1.42~1.87)×10^(-3) mm^(2)/s,ADC平均值1.65×10^(-3) mm^(2)/s,SUVmax范围1.52~11.4,SUVmax平均值6.46,3例肺肉瘤样癌T2WI信号均低于2例肺腺鳞癌,ADC平均值小于2例肺腺鳞癌,SUVmax平均值大于2例肺腺鳞癌。5例病灶均位于右肺上叶,其中3例肺肉瘤样癌病灶均为肿块且位于纵隔旁,2例肺腺鳞癌病灶均见浅分叶,1例肺肉瘤样癌伴远端肺阻塞。结论 ^(18)F-FDG PET/MR对肺部罕见恶性肿瘤的诊断和鉴别诊断具有一定价值。
Objectivee To explore the diagnostic value of 18 F-fluorodeoxyglucose(FDG)positron emission tomography(PET)/MR multimodal imaging in rare pulmonary malignancies.Methods The clinical,^(18)F-FDG PET/MR and pathological data of 5 patients with rare pulmonary malignancies confirmed by pathology from June 2019 to February 2024 were analyzed retrospectively,including tumor quantity,location,shape,size,sign,signal,peripheral invasion,systemic metastasis,apparent diffusion coefficient(ADC)and standard uptake value(SUV)were analyzed.Results 5 cases of lesions showed high signal intensity on DWI sequence.In 3 cases of pulmonary sarcomatoid carcinoma,the range of ADC value was(0.670~1.812)×10^(-3) mm²/s,the average value of ADC was 1.168 X ×10^(-3) mm^(2)/s,the range of SUVmax was 3.96~35.23,and the average value of SUVmax was 10.36.In 2 cases of lung adenosquamous carcinoma,the range of ADC value was(0.939~1.820)X 10-3 mm²/s,and the average value of ADC was 1.380 X10-3mm²/s,the range of SUVmax 1.52~11.4,and the average value of SUVmax was 6.46.The T2WI signal of 3 cases of pulmonary sarcomatoid carcinoma was lower than that of 2 cases of lung adenosquamous carcinoma,the average value of ADC was less than 2 cases of lung adenosquamous carcinoma,and the average value of SUVmax was higher 2 cases of pulmonary adenosquamous carcinoma.5 cases were all located in the upper lobe of the right lung,3 cases of pulmonary sarcomatoid carcinoma were masses and adjacent to the mediastinum,2 cases of lung adenosquamous carcinoma showed superficial lobulation,and 1 case of pulmonary sarcomatoid carcinoma with distal lung obstruction.Conclusion ^(18)F-FDG PET/MR has a certain value in the diagnosis and differential diagnosis of rare pulmonary malignancies.
