摘要
目的观察重组人脑利钠肽联合硝酸甘油治疗老年高血压伴心力衰竭的疗效及对心功能与心肌损伤标志物的影响。方法选取2022年8月—2024年8月南昌市人民医院收治的老年高血压伴心力衰竭患者90例,采用便利抽样法随机分为研究组和对照组,每组45例。2组入院后均进行常规治疗,对照组给予硝酸甘油治疗,研究组在对照组的基础上给予重组人脑利钠肽治疗,2组均连续治疗3 d。比较2组临床疗效,治疗前后心功能指标[左室射血分数(LVEF)、左室舒张末期内径(LVEDD)、左室收缩末期内径(LVESD)]、心肌损伤标志物[肌红蛋白(MYO)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白I(cTnI)]。结果研究组总有效率为95.56%,高于对照组的80.00%(χ^(2)=5.075,P=0.024)。治疗3 d后,2组LVEF均较治疗前升高,LVEDD和LVESD均较治疗前缩小,且研究组变化幅度大于对照组(P<0.05或P<0.01);2组MYO、CK-MB、cTnI水平均较治疗前降低,且研究组降低幅度大于对照组(P<0.05或P<0.01)。结论重组人脑利钠肽联合硝酸甘油治疗老年高血压伴心力衰竭可提高临床效果,有效改善患者心功能和心肌损伤标志物。
Objective To observe the efficacy of recombinant human brain natriuretic peptide combined with nitroglycerin in the treatment of elderly hypertension with heart failure and its effect on cardiac function and myocardial injury indicators.Methods A total of 90 elderly patients with hypertension and heart failure treated in Nanchang People′s Hospital from August 2022 to August 2024 were randomly divided into study group and control group by convenient sampling method,with 45 cases in each group.After admission,the two groups received conventional treatment,the control group was given nitroglycerin treatment,the study group was given recombinant human brain natriuretic peptide treatment on the basis of the control group,and the two groups were treated continuously for 3 days.The clinical efficacy,acardiac function indexes(LVEF,LVEDD,LVESD),myocardial injury indexes(MYO,CK-MB,cTnI)before and after treatment were compared.Results The total effective rate of the study group was 95.56%,which was higher than that of the control group(80.00%)(χ^(2)=5.075,P=0.024).After 3 days of treatment,LVEF in two groups increased,LVEDD and LVESD in two groups decreased,and the change range of the study group was greater than that of the control group(P<0.05 or P<0.01).The levels of MYO,CK-MB and cTnI in two groups were lower than those before treatment,and the reduction in the study group was greater than that in the control group(P<0.05 or P<0.01).Conclusion Recombinant human brain natriuretic peptide combined with nitroglycerin in the treatment of elderly hypertension with heart failure can improve the clinical effect,effectively improve the heart function and myocardial injury indexes.
