摘要
代谢相关性脂肪性肝病(MAFLD)是一种机体代谢紊乱引起的肝脏脂肪变性及其进展导致的炎症、纤维化的慢性肝脏疾病,与肝硬化、肝细胞癌、肥胖、2型糖尿病、心血管疾病等代谢性疾病的发生发展密切相关。脂肪组织功能障碍介导的代谢灵活性降低、炎症及脂肪因子分泌异常是促进MAFLD进展的关键。不同类型脂肪组织在MAFLD的发生发展中发挥不同作用。大量研究证实,棕色脂肪组织可通过增强机体胰岛素敏感性、调节脂质代谢而改善代谢相关性疾病。因此,减少白色脂肪在体内过多的积累,调节白色脂肪棕色化可能是治疗MAFLD的重要措施。本文以脂肪组织为中心,就脂肪组织功能障碍在MAFLD发生发展中的作用和具体机制,以及当前靶向脂肪组织功能障碍的各种疗法的最新进展和遇到的主要问题与挑战展开综述。
Metabolic associated fatty liver disease(MAFLD)is a chronic liver disease characterized by hepatic steatosis caused by metabolic disorders,and its progression leading to inflammation and fibrosis of the liver.It is closely associated with the development of metabolic diseases such as cirrhosis,hepatocellular carcinoma,obesity,type 2 diabetes,and cardiovascular diseases.Reduced metabolic flexibility,inflammation,and abnormal secretion of adipokines mediated by adipose tissue dysfunction are key factors driving the progression of MAFLD.Different types of adipose tissues play distinct roles in the pathogenesis of MAFLD.Substantial studies have confirmed that brown adipose tissue can ameliorate metabolic disorders by enhancing systemic insulin sensitivity and regulating lipid metabolism.Therefore,reducing the excessive accumulation of white adipose tissue and regulating the browning of white adipose tissue might serve as critical strategies for MAFLD treatment.This review focuses on adipose tissue,elucidating the effects and mechanisms of adipose tissue dysfunction on the development of MAFLD,as well as summarizing the latest advances,challenges,and major issues in current therapies targeting adipose tissue dysfunction.
