摘要
目的探讨KTH模式护理对腹膜透析患者依从性的影响,以改善治疗效果和患者的生活质量。方法选取2022年6月—2023年12月天津医科大学第二医院肾内科收治的94例新置管接受腹膜透析患者,随机分为对照组和研究组各47例。对照组给予常规护理,研究组给予KTH模式护理。两组均随访3个月。比较两组护理前后心理情绪变化、BUN、CRE、HGB及治疗依从性。结果护理前,两组HAMA、HAMD评分比较,差异均无统计学意义(P>0.05);护理后,研究组HAMA、HAMD评分明显低于对照组,差异均有统计学意义(P<0.05)。护理前,两组BUN、CRE、HGB比较,差异均无统计学意义(P>0.05);护理后,研究组BUN、CRE水平明显低于对照组,且HGB水平高于对照组,差异均有统计学意义(P<0.05)。护理前,两组用药、液体摄入、透析方案依从性评分比较,差异均无统计学意义(P>0.05);护理后,研究组用药、液体摄入、透析方案依从性评分明显高于对照组,差异均有统计学意义(P<0.05)。结论KTH模式护理对腹膜透析患者有积极的影响,不仅取得了明显的透析成效,有效清除患者体内的代谢废物,还在心理功能和治疗依从性改善方面发挥了重要作用,值得临床借鉴推广。
Objective To explore the influence of KTH mode nursing on the compliance of peritoneal dialysis patients,so as to improve the treatment effect and the quality of life of patients.Methods From June 2022 to December 2023,94 patients who received peritoneal dialysis with new catheter were selected and randomly divided into control group and study group,with 47 cases in each group.The control group was given routine nursing,while the study group was given KTH mode nursing.Both groups were followed up for 3 months,the psychological and emotional changes,BUN,CRE,HGB and treatment compliance of the two groups were compared before and after nursing.Results Before nursing,there were no significant differences in HAMA and HAMD scores between the two groups(P>0.05);after nursing,the scores of HAMA and HAMD in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Before nursing,there were no significant differences in BUN,CRE and HGB between the two groups(P>0.05);after nursing,the levels of BUN and CRE in the study group were significantly lower than those in the control group,and the level of HGB was higher than that in the control group,and the differences were statistically significant(P<0.05).Before nursing,there were no significant differences in medication,fluid intake and dialysis plan compliance scores between the two groups(P>0.05);after nursing,the compliance scores of medication,fluid intake and dialysis plan in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05).Conclusion KTH model nursing had a positive effect on peritoneal dialysis patients,which not only achieved remarkable dialysis effect and effectively removed metabolic waste from patients,but also played an important role in improving psychosocial function and treatment compliance,and was worthy of clinical reference and promotion.
