孕11~13^(+6)周NT值联合三维超声诊断胎儿畸形效果 被引量:1

Effect of the fetal nuchal translucency value combined with the three-dimensional ultrasound for diagnosing fetal malformation during 11-13^(+6) gestational weeks

摘要 目的:探究孕11~13^(+6)周NT值联合三维超声对胎儿畸形的诊断价值。方法:选取2020年5月-2023年5月在本院进行早孕期产前筛查的孕11~13^(+6)周孕妇90例临床资料,均行二维、三维超声检查并测量颈项透明层(NT)值,采用四格表评估二维和三维超声诊断胎儿畸形的特异度、灵敏度和准确率,采用受试者工作特征(ROC)曲线分析NT值联合三维超声诊断价值。结果:二维和三维超声对胎儿畸形的诊断灵敏度分别为40.0%、60.0%,特异度分别为97.7%、98.8%,准确率分别为94.3%、96.7%,阳性预测值分别为50.0%、75.0%,阴性预测值分别为96.5%、97.7%;NT值胎儿畸形组(3.14±0.74mm)大于胎儿无畸形组(2.25±0.68mm)(P>0.05);ROC曲线分析显示,NT值联合三维超声诊断胎儿畸形的曲线下面积为0.767(95%CI 0.554~0.980),灵敏度80.0%、特异度68.2%。结论:孕11~13^(+6)周NT值联合三维超声对胎儿畸形有良好的诊断价值和诊断灵敏度。 Objective:To explore the value of the fetal nuchal translucency(NT)value combined with the three-dimensional ultrasound for diagnosing fetal malformation during 11-13^(+6) gestational weeks.Methods:The clinical data of 90 pregnant women who had received the prenatal screening in the hospital during 11-13+6 gestational weeks from May 2020 to May 2023 were selected in this study.These women were exanimated by two-dimensional and three-dimensional ultrasound and the fetal NT value of these women was measured.The four-fold table was used to evaluate the specificity,the sensitivity and the accuracy rate of two-dimensional or three-dimensional ultrasound for diagnosing the fetal malformation.The diagnostic value of the fetal NT value combined with the three-dimensional ultrasound examination of the women for diagnosing their fetal malformation was analyzed by receiver operator characteristic(ROC)curve.Results:The sensitivities of two-dimensional ultrasound examination and three-dimensional ultrasound examination of the women for diagnosing their fetal malformation were 40.0% and 60.0%,the specificities of which were 97.7% and 98.8%,the accuracy rates of which were 94.4% and 96.7%,the positive predictive values of which were 50.0% and 75.0% and the negative predictive values of which were 96.5% and 97.7%.The NT value(3.14±0.74mm)of the fetuses with malformations was significantly higher than that(2.25±0.68mm)of the fetuses without malformations(P>0.05).ROC curve analysis showed that the area under the curve,the sensitivity and the specificity of the fetal NT value combined with the results of the three-dimensional ultrasound of the pregnant women for diagnosing their fetal malformations were 0.767(95%CI 0.554-0.980),80.0% and 68.2%,respectively.Conclusion:The fetal NT value combined with the results of the three-dimensional ultrasound of the pregnant women during 11-13+6 gestational weeks for diagnosing their fetal malformations has good diagnostic value and diagnostic sensitivity.
