摘要
目的 探讨产前三维联合二维超声检查对孕中期胎儿畸形筛查的诊断优势和价值。方法 选择2017年1月至2024年2月河南科技大学第二附属医院收治的经分娩或引产证实为胎儿畸形的273例产妇为研究对象,另选择同期分娩证实为健康胎儿的产妇337例。应用二维及三维超声进行诊断,将结果与分娩、引产结果进行比较,统计诊断准确率,分析诊断价值。结果 三维超声联合二维超声在诊断胎儿畸形、健康胎儿上准确率均高于二维超声,差异有统计学意义(P<0.05),三维超声联合二维超声诊断肢体畸形、肋骨、面部畸形、中枢神经系统准确率高于二维超声,差异有统计学意义(P<0.05)。结论 三维超声联合二维超声用于孕中期筛查及诊断胎儿畸形中效果更好,可提高总体检出率及准确率,尤其是对四肢短小或四肢缺失、手裂、足内翻伴或不伴多(并)趾指畸形的诊断价值。
Objective To investigate the diagnostic advantages and value of prenatal threedimensional combined with two-dimensional ultrasound in screening fetal deformities in mid pregnancy compared to two-dimensional ultrasound.Methods A total of 273 maternal patients admitted to the Second Affiliated Hospital of Henan University of Science and Technology from January 2017 to February 2024 were selected to participate in the study.In the same period,337 women chose to give birth and proved to be healthy fetuses.The patients were diagnosed by two-dimensional and three-dimensional ultrasound,and the results were compared with the results of labor and induced labor,and the diagnostic accuracy and diagnostic value were analyzed.Results The accuracy of 3D ultrasound combined with 2D ultrasound in diagnosing fetal malformations and healthy fetuses was higher than that of 2D ultrasound(P<0.05),and the accuracy of 3D ultrasound combined with 2D ultrasound in diagnosing limb malformations,ribs,facial malformations and central nervous system was higher than that of 2D ultrasound(P<0.05).Conclusions Three-dimensional ultrasound combined with two-dimensional ultrasound is more effective on screening and diagnosis of fetal malformation in the second trimester,which can improve the overall detection rate and accuracy,especially for the diagnosis of short limbs or missing limbs,hand cleft,foot pronation with or without multiple(and)dactylophaly.
