摘要
目的:通过数据挖掘技术,探讨朱俊琛教授临床辨治下腰痛处方用药及选穴规律与临证心得。方法:收集朱俊琛教授临床诊治下腰痛病历172例,提取方药信息,建立数据库,采用古今医案云平台V2.3.7,对下腰痛中药组方和针灸取穴采用中医证候分析、药对分析、关联规则分析、聚类分析、复杂网络分析等进行数据挖掘。结果:172首处方,中医证型4个,中药87味,总用药频次为2737次,共纳入30个穴位,总计2444频次。中药频次前4位的高频单药分别为炙甘草、细辛、熟地黄、盐杜仲;穴位频次前3位的为肾俞、腰阳关、次髎;中药功效前3位的为祛风止痛、通络、祛风除湿;四气以温、平为主,五味以甘、辛为主,药物多归肝、肾、脾三经;中药聚类分析得出聚类组合8类;中药关联分析显示,居于前3位的分别为细辛-炙甘草、乌梢蛇-炙甘草、炮附片-炙甘草。核心处方为“肾俞-腰阳关-次髎-稚边-委中-承山-太溪-昆仑”。复杂网络分析结果显示,肾俞、腰阳关、次髎和炙甘草、细辛、熟地黄等中药关联最为密切。结论:朱俊琛教授治疗下腰痛以祛风除湿、通络止痛、补肾壮骨为主要法则,临床取穴注重表里相治、上下同调。
Objective:To discusses Professor Zhu Junchen's clinical prescription medication and acupoint selection rules and his clinical experience in the syndrome differentiation and treatment of lower back pain through data mining technology.Methods:A total of 172 cases of low back pain were collected from Prof.Zhu Junchen's clinical diagnosis and treatment,and the prescription information was extracted to establish a database.The ancient and modern medical case cloud platform V2.3.7 was used to perform data mining on the traditional Chinese medicine formulas and acupuncture acupoints selection for low back pain using TCM syndrome analysis,drug pair analysis,association rule analysis,cluster analysis and complex network analysis.Results:There were 172 prescriptions,4 types of traditional Chinese medicine syndromes,87 kinds of Chinese herbal medicines,and the total frequency of medication was 2737 times.A total of 30 acupoints were included,with a total frequency of 2444 times.The top four high frequency single Chinese herbs were Zhigancao,Xixin,Shudihuang and Yanduzhong;the top three acupoints in terms of frequency were Shenshu,Yaoyangguan and Ciliao;the top three effects of Chinese herbs were dispelling wind and relieving pain,dredging collaterals,dispelling wind and removing dampness;the four natures were mainly warm and mild,the five flavors were mainly sweet and pungent,and the herbs were mostly attributed to the liver,kidney and spleen meridians;cluster analysis of Chinese herbs showed 8 types of cluster combinations;according to the association analysis of Chinese herbs,the top three were Xixin-Zhigancao,Wushaoshe-Zhigancao,and Paofupian-Zhigancao.The core prescription was“Shenshu-Yaoyangguan-Ciliao-Zhibian-Weizhong-Chengshan-Taixi-Kunlun”.The results of complex network analysis showed that Shenshu,Yaoyangguan and Ciliao were most closely related to Chinese herbs such as Zhigancao,Xixin and Shudihuang.Con⁃clusion:Prof.Zhu Junchen's treatment of low back pain is mainly based on the principles of dispelling wind and removing dampness,dredging collaterals and relieving pain,tonifying kidney and strengthening bone.In clinical acupoint selection,he pays attention to treating both the exterior and interior aspects of the body,as well as regulating the upper and lower parts simultaneously.
