摘要
目的探讨双环醇联合门冬氨酸鸟氨酸颗粒对失代偿期肝硬化(decompensated cirrhosis,DCC)患者临床疗效及肝功能的影响。方法选取本院2021年10月至2022年12月确诊的DCC患者80例,采用简单随机化法将患者分为对照组和研究组,每组40例。对照组予双环醇,研究组予双环醇联合门冬氨酸鸟氨酸颗粒,两组均连续治疗3周。比较两组临床效果、肝功能、肝纤维化、血清炎性因子、T淋巴细胞以及不良反应发生情况。结果治疗后,研究组总有效率高于对照组(92.50%vs.75.00%,P<0.05)。两组总胆红素、天冬氨酸氨基转移酶、丙氨酸氨基转移酶、白介素(interleukin,IL)-2、IL-6、肿瘤坏死因子-α、C反应蛋白、层黏连蛋白以及透明质酸较前均降低,且研究组低于对照组(P<0.05)。两组CD3^(+)、CD4^(+)/CD8^(+)以及CD4^(+)水平较前均显著提高,且研究组高于对照组(P<0.05)。研究组不良反应发生率与对照组比较,差异无统计学意义(5.00%vs.12.50%,P>0.05)。结论双环醇联合门冬氨酸鸟氨酸颗粒治疗DCC,可有效改善患者的临床症状,提高临床疗效,改善肝功能和免疫能力,减轻炎性反应,降低肝硬化程度。
Objective To investigate the clinical efficacy and impact on liver function of bicyclol combined with ornithine aspartate granules in patients with decompensated cirrhosis(DCC).Methods A total of 80 DCC patients diagnosed in our hospital from October 2021 to December 2022 were selected and divided into control group and study group by simple randomization method,with 40 patients in each group.The control group was treated with dicyclol,and the study group was treated with dicyclol combined with ornithine aspartate granules.Both groups were treated continuously for 3 weeks.The clinical effect,liver function index,liver fibrosis index,serum levels of inflammatory cytokines,T lymphocytes and the occurrence of adverse reactions were compared between two groups after treatment.Results After treatment,the total effective rate of the study group was higher than of the control group(92.50%vs.75.00%,P<0.05).Total bilirubin,aspartate aminotransferase,alanine aminotransferase,interleukin(IL)-2,IL-6,tumor necrosis factor-α,C-reactive protein,laminin,and hyaluronic acid decreased in both groups compared with those before treatment,and the study group was lower than the control group(P<0.05).The levels of CD3^(+),CD4^(+)/CD8^(+)and CD4^(+)in bothgroups were significantly higher than before treatment,and the study group was higher than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the study group and the control group(5.00%vs.12.50%,P>0.05).Conclusion In the treatment of DCC patients,dicyclol combined with ornithine aspartate granules can effectively improve clinical symptoms,improve clinical efficacy,improve liver function and immune capacity,reduce inflammatory response,and reduce the degree of cirrhosis.
