摘要
目的比较食管癌证素特点、组合规律和证型的临床与文献研究结果,为食管癌证型标准的更新修订提供研究依据。方法拟定证候调查表进行临床调查,文献整理了食管癌病因病机、证候、证或证治的内容,从病位和病性证素、高频证素聚类和因子分析、组合证型等多方面对比二者的研究结果,并与传统教材的证型进行比较分析。结果临床获5个高频病位证素和9个高频病性证素;文献获6个高频病位证素和14个高频病性证素;临床高频证素经过聚类、因子分析后可形成4种证型,文献高频证素经聚类、因子分析后可形成6种证型。结论食管癌的病位在脾、胃、肝、肺、肾;以气滞、痰、血瘀、湿、气虚、血虚、阳虚、阴虚为基本病机;在传统教材的证型基础上,增补了毒热蕴结、湿食结滞、气滞阴(血)虚3个证型,并对部分基本证型增加了亚型,丰富了对食管癌证型的认识。
Objective To compare the clinical and literature research results of the characteristics,combination rule and syndromes of esophagus cancer syndrome factors,so as to provide research basis for the updating and revision of esophageal cancer syndrome type standards.Methods The syndrome questionnaire of clinical investigation was formulated and researched,and in literature research contents of esophageal cancer etiology and pathogenesis,and syndrome or treatment syndrome were retrieved and sorted out,the research results of both from the location syndrome factors and etiological syndrome factors,high frequency factor clustering and factor analysis,combination syndrome types were compared,and the difference with the traditional teaching material content were analyzed.Results In clinical practice,there were 5 location syndrome factors and 9 etiological syndrome factors of high frequency.In literature,there were 6 location syndrome factors and 14 etiological syndrome factors of high frequency.In clinical practice,four types of syndromes could be formed by clustering analysis of high-frequency syndromes and factor analysis.While in literature,six types of syndromes could be formed by clustering analysis of high-frequency syndromes and factor analysis.Conclusion The disease location of esophageal cancer is spleen and stomach,liver,lung and kidney.The basic pathogenesis is qi stagnation,phlegm,blood stasis,dampness,qi deficiency,blood deficiency,yang deficiency and yin deficiency.On the basis of the syndrome types in the traditional teaching materials,three syndrome types of toxic heat accumulation,wet and food stagnation,qi stagnation and yin(blood)deficiency are added,and some basic syndrome types are added,so as to renew the rich understanding of esophageal cancer syndrome types.
