摘要
基于仝小林院士所创“态靶辨证”这一特色理论,探讨盐敏感性高血压(SSHT)的发病机制及辨治思路。认为“水饮内停,瘀血阻络”为SSHT的核心病机,与现代医学的SSHT水钠潴留及血管内皮损伤的发病机制相吻合。在中医学宏观辨证识态的基础上,结合现代药理学研究成果微观辨病定靶,提出以“水停瘀阻”为态,“眩晕、水肿、高血压、一氧化氮(NO)减少”为靶,以五苓散为靶方通阳化气、利水蠲饮,茺蔚子、丹参、泽兰活血祛瘀、利水消肿为靶药,化气行水、活血通脉,态靶同调,实现除因降压、辨证降压、对症降压,为临床运用中医药治疗SSHT提供参考与借鉴。
Based on the unique theory of“state-target syndrome differentiation”created by Academician TONG Xiaolin,this study explores the pathogenesis and treatment strategies for salt-sensitive hypertension(SSHT).It is proposed that the core pathogenesis of SSHT is“water retention and static blood obstructing collaterals”,which aligns with the modern medical understanding of SSHT involving water-sodium retention and vascular endothelial injury.On the basis of macro-level syndrome differentiation and state identification in traditional Chinese medicine(TCM),combined with modern pharmacological findings of micro-level disease differentiation for identifying targets,the theory regards“water retention and static blood obstruction”as the state and symptoms such as dizziness,edema,hypertension,and reduced nitric oxide(NO)levels as the targets.Wuling Powder is recommended as target formula to promote yang,transform qi,dispel water,and alleviate water retention.Leonuri Fructus,Salviae Miltiorrhizae Radix et Rhizoma,and Lycopi Herba can activate blood,resolve stasis,promote diuresis,and reduce swelling,serving as target drugs.This integrated approach can achieve the effects of transforming qi,moving water,activating blood,and dredging collaterals,thereby simultaneously regulating state and targets.This method can eliminate the root cause,regulate blood pressure based on syndrome differentiation,and provide symptomatic relief,offering a reference for clinical application of TCM in treating SSHT.
