摘要
目的:对特发性肺纤维化肺康复的有效性及安全性进行系统评价再评价,为临床工作者肺康复方案的制订提供循证依据。方法:计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库等中文数据库以及Cochrane Library、PubMed、Embase、CINAHL等数据库有关特发性肺纤维化肺康复的系统评价,并补充检索灰色文献。检索时间截止至2024年5月,由2位研究者独立完成纳入文献的质量评价、资料提取。结果:共纳入8篇系统评价,其中2篇为高质量,1篇为中质量,1篇为低质量,4篇为极低质量文献。在结局指标方面,共评价了31个结局指标,其中仅5个(16.13%)评价为中等质量证据,其余为低或极低质量证据,总体质量偏低。结论:肺康复对IPF患者是有益且安全的,能提高患者运动耐力、生活质量,改善呼吸困难症状,延缓肺功能下降;但对于肺康复干预后的远期效果有待于进一步验证。
Objective:To reevaluate the effectiveness and safety of pulmonary rehabilitation for idiopathic pulmonary fibrosis,and to provide an evidence-based basis for the development of pulmonary rehabilitation programs for clinical workers.Methods:Computer searches were performed on Chinese databases such as CNKI Datebase,Wanfang Database,VIP Database,and China Biomedical Literature Database,as well as Cochrane Library,PubMed,Embase,and CINAHL databases on systematic evaluation of pulmonary rehabilitation for idiopathic pulmonary fibrosis and supplemented with searches of gray literature.The search was conducted until May 2024,and two researchers independently completed the quality evaluation and information extraction of the included literature.Results:A total of eight systematic evaluations were included,of which two were high-quality,one was medium-quality,one was low-quality,and four were very-low-quality literature.In terms of outcome indicators,a total of 31 outcome indicators were evaluated,of which only 5(16.13%)were evaluated as moderate-quality evidence,and the rest as low-or very low-quality evidence,with an overall low quality.Conclusion:Pulmonary rehabilitation is beneficial and safe for patients with IPF,and can improve exercise tolerance,quality of life,dyspnea symptoms,and delay the decline of lung function.However,further validation is needed for the long-term effects after pulmonary rehabilitation intervention.
