摘要
目的:系统评价临床常用中医护理干预对老年高血压病人血压的影响。方法:检索中国期刊全文数据库(CNKI)、维普数据库(VIP)、万方数据库(WanFang Data)以及PubMed、Web of Science、the Cochrane Library数据库中关于灸法、穴位按摩、八段锦、中医耳穴疗法、刮痧、太极拳、中药穴位贴敷7种护理干预对比常规治疗在改善老年高血压病人血压效果方面的随机对照试验(RCT),文献检索时限为建库至2023年3月10日。进行文献筛选、资料提取并评价纳入研究的偏倚风险,采用网状Meta分析方法及曲线下累积排序概率面积进行有效性比较和排序。结果:最终纳入32篇文献。网状Meta分析结果显示,7种中医护理干预中,穴位按摩改善老年高血压病人血压的效果最好;在收缩压指标上,效果排名前3位的依次为常规治疗+穴位按摩、常规治疗+中药穴位贴敷、常规治疗+刮痧;在舒张压指标上,效果排名前3位的依次为常规治疗+穴位按摩、常规治疗+灸法、常规治疗+中药穴位贴敷;在降低中医证候积分上,刮痧效果最好。结论:现有证据显示,常用中医护理干预在改善老年高血压病人血压中各有特点,穴位按摩在改善老年高血压病人血压方面效果最好。
Objective:To systematically evaluate the effects of clinical commonly used Chinese medicine nursing intervention on blood pressure in elderly patients with hypertension.Methods:RCT articles published in CNKI,VIP,WanFang Data,PubMed,Web of Science and the Cochrane Library on improving hypertension of elderly patients referring the comparing 7 kinds of nursing intervention including moxibustion,acupressure,Baduanjin,traditional Chinese medicine auricular therapy,Guasha,Tai Chi and Chinese herbal acupoint application with routine treatment were retrieved.And retrieval time was from database establishment to March 10,2023.Literature screening,data extraction,and evaluation of the risk of bias in the included studies were carried out.The network Meta-analysis and surface under the cumulative ranking curve area were used to compare and rank the effectiveness.Results:A total of 32 articles were ultimately included.The results of the network Meta-analysis showed that among the 7 traditional Chinese medicine nursing intervention,acupressure had the best effect on improving blood pressure in elderly patients with hypertensive.In terms of systolic blood pressure control,the top three in terms of effectiveness were conventional treatment+acupressure,conventional treatment+Chinese herbal acupoint application,and conventional treatment+Guasha in order.In terms of diastolic blood pressure control,the top three in terms of effectiveness were conventional treatment+acupressure,conventional treatment+moxibustion,and conventional treatment+Chinese herbal acupoint application in order.In terms of reducing the score of traditional Chinese medicine symptom complex,Guasha had the best effect.Conclusions:Existing evidences showed that clinical commonly used Chinese medicine nursing intervention had their own characteristics in improving blood pressure of elderly patients with hypertension.And acupressure has the best effect in improving blood pressure in elderly patients with hypertension.
