摘要
目的分析肺功能康复护理联合营养支持对慢性阻塞性肺疾病(COPD)合并2型糖尿病(T2DM)患者肺功能的影响。方法按抽签法将福建省宁化县医院2021年2月至2023年2月收治的100例COPD合并T2DM患者分为对照组与观察组,各50例。对照组患者采用常规方式进行护理,观察组患者采用肺功能康复护理联合营养支持。比较护理前后组间肺功能、生活质量[健康调查量表36(SF-36)]、血糖水平及营养状况。结果护理1个月后,观察组的动脉血二氧化碳分压、动脉血氧分压、一秒率和第1秒用力呼气容积占预计值的百分比均优于对照组(P均<0.05)。护理1个月后,观察组的空腹血糖、餐后2 h血糖和糖化血红蛋白水平均低于对照组(P均<0.05);躯体功能等4项SF-36评分,以及白蛋白和前白蛋白水平均高于对照组(P均<0.05)。结论肺功能康复护理联合营养支持能改善COPD合并2型糖尿病患者的肺功能、血糖水平及营养状况,提高其生活质量。
Objective To analyze the effect of pulmonary function rehabilitation nursing combined with nutritional support on the pulmonary function of patients with chronic obstructive pulmonary disease(COPD)and type 2 diabetes mellitus(T2DM).Methods A total of 100 COPD patients with T2DM who were admitted to Ninghua County General Hospital from February 2021 to February 2023 were selected as the subjects of this study.The subjects were assigned into control and observation groups by drawing lots,each with 50 cases.The control group received routine nursing,while the observation group received pulmonary function rehabilitation nursing combined with nutritional support.The study compared the pulmonary function,quality of life(Short Form-36 Health Survey[SF-36]),blood glucose levels,and nutritional status between the two groups before and after nursing.Results After 1 month of nursing,the observation group had lower arterial partial pressure of carbon dioxide,higher arterial partial pressure of oxygen,higher forced expiratory volume in 1 second/forced vital capacity,and higher percentage of predicted forced expiratory volume in 1 second compared with the control group(P<0.05).After 1 month of nursing,the fasting blood glucose,2-h postprandial blood glucose,and glycosylated hemoglobin levels in the observation group were all lower than those in the control group(P<0.05).The SF-36 scores such as physical function,as well as the albumin and prealbumin levels,were all higher than those in the control group(P<0.05).Conclusion Pulmonary function rehabilitation nursing combined with nutritional support can improve the pulmonary function,blood glucose levels,nutritional status,and quality of life of COPD patients with T2DM.
