577 nm微脉冲激光与激光光凝疗法治疗糖尿病视网膜病变的疗效分析

摘要 目的探讨577 nm微脉冲激光与激光光凝疗法治疗糖尿病视网膜病变(diabetic retinopathy,DR)的疗效。方法选取2021年5月至2022年12月南昌市第一医院收治的60例(120只眼)DR患者作为研究对象,按照随机数字表法分为对照组与观察组,每组30例(60只眼)。对照组行激光光凝疗法治疗,观察组行577 nm微脉冲激光治疗。比较两组临床疗效,治疗前后视力水平及视网膜功能、血清指标[血管内皮生长因子(vascular endothelial growth factor,VEGF)、促红细胞生成素(erythropoietin,EPO)及白细胞介素-6(interleukin 6,IL-6)]及并发症发生情况。结果观察组治疗总有效率为96.67%,高于对照组的73.33%,差异有统计学意义(P<0.05)。治疗后,两组黄斑区视网膜厚度均薄于治疗前,黄斑中心凹容积均小于治疗前,视网膜周边光敏度均高于治疗前,观察组最佳矫正视力行数多于治疗前,且观察组黄斑区视网膜厚度薄于对照组,黄斑中心凹容积小于对照组,视网膜周边光敏度低于对照组,最佳矫正视力行数多于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清VEGF、IL-6水平均低于治疗前,观察组EPO水平低于治疗前,且观察组VEGF、EPO、IL-6水平均低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率为3.33%,低于对照组的26.67%,差异有统计学意义(P<0.05)。结论577 nm微脉冲激光治疗DR疗效显著,可改善患者视力水平及视网膜功能,降低并发症发生率,值得临床推广应用。 Objective To investigate the therapeutic efficacy of 577 nm micropulse laser and laser photocoagulation in the treatment of diabetic retinopathy(DR).Methods 60 cases(120 eyes)patients with DR admitted to The First Hospital of Nanchang from May 2021 to December 2022 were selected as the study subjects,and they were divided into the control group and the observation group according to the random number table method,with 30 cases(60 eyes)in each group.The control group was treated with laser photocoagulation,and the observation group was treated with 577 nm micropulse laser.The clinical efficacy,visual acuity and retinal function,serum indexes(vascular endothelial growth factor[VEGF],erythropoietin[EPO]and interleukin-6[IL-6])before and after treatment and the occurrence of complications were compared between the two groups.Results The total effective rate in the observation group was 96.67%,which was higher than 73.33%in the control group,and the difference was statistically significant(P<0.05).After treatment,the macular retinal thickness of the two groups were thinner than before treatment,macular central concave volume were smaller than before treatment,the peripheral light sensitivity were higher than before treatment,lines of best-corrected visual acuity in the observation group was more than before treatment,and the macular retinal thickness in the observation group was thinner than that in the control group,the macular central concave volume was smaller than that in the control group,the peripheral light sensitivity was lower than that in the control group,and the lines of best-corrected visual acuity was more than that in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of serum VEGF and IL-6 of the two groups were lower than before treatment,the EPO levels in the observation group was lower than before treatment,and levels of serum VEGF,EPO and IL-6 in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The complication rate in the observation group was 3.33%,which was lower than 26.67%in the control group,and the difference was statistically significant(P<0.05).Conclusion 577 nm micropulse laser treatment for patients with DR has the significant effect,it can improve their visual acuity level and retinal function,and reduce the complication rate,and is worthy of clinical promotion and application.
