摘要
目的分析妊娠期糖尿病(GDM)应用胰岛素泵联合维生素D(VD)治疗的效果。方法队列研究。抽取2021年7月至2023年7月渭南市妇幼保健院收治的109例GDM患者,按照随机数字表法分为对照组(54例)和研究组(55例)。对照组行胰岛素泵治疗,研究组在对照组基础上联合维生素D治疗,比较两组血糖、血脂、胰岛β细胞功能及妊娠结局。结果治疗后,两组空腹血糖、餐后2 h血糖及糖化血红蛋白水平均低于治疗前,且研究组低于对照组(P<0.05)。治疗后,两组低密度脂蛋白胆固醇、三酰甘油、总胆固醇、胰岛素、胰高血糖素、胰岛素抵抗指数水平均低于治疗前,且研究组低于对照组(P<0.05)。治疗后,两组高密度脂蛋白胆固醇、胰岛β细胞功能指数水平均高于治疗前,且研究组高于对照组(P<0.05);研究组不良妊娠结局发生率(5.45%,3/55)低于对照组(20.37%,11/54),P<0.05。结论GDM应用胰岛素泵联合维生素D治疗,可改善糖脂代谢,促进胰岛β细胞功能改善,并改善妊娠结局。
Objective To analyze the effect of insulin pump combined with vitamin D(VD)in the treatment of gestational diabetes mellitus(GDM).Methods A total of 109 GDM patients admitted to Weinan Maternal and Child Health Hospital from July 2021 to July 2023 were selected for the cohort study.And they were divided into the control group(54 cases)and the study group(55 cases)using a random number table method.The control group was treated by insulin pump,while the study group was treated with vitamin D based on the treatment of the control group.The blood glucose,blood lipid,pancreaticβ-cell function and pregnancy outcome were compared between the two groups.ResultsAfter treatment,the levels of fasting blood glucose,2-hour postprandial blood glucose and glycosylated hemoglobin in the two groups were lower than those before treatment,and the above indexes in the study group were lower than those in the control group(P<0.05).After treatment,the levels of low density lipoprotein cholesterol,triglyceride,total cholesterol,insulin,glucagon and insulin resistance index in the two groups were lower than those before treatment,and the above indexes in the study group were lower than those in the control group(P<0.05).After treatment,the levels of high density lipoprotein cholesterol and pancreaticβ-cell function index in the two groups were higher than those before treatment,and the above indexes in the study group were higher than those in the control group(P<0.05).The incidence of adverse pregnancy outcome in the study group(5.45%,3/55)was lower than that in the control group(20.37%,11/54),P<0.05.Conclusions The insulin pump combined with vitamin D in the treatment of GDM can ameliorate glucose and lipid metabolism,promote the alleviation of pancreaticβ-cell function,and improve pregnancy outcome.
