非体外循环与体外循环冠状动脉搭桥术治疗冠心病患者的临床疗效观察

Clinical Efficacy of Off-Pump and Extracorporeal Circulation Coronary Artery Bypass Grafting in Patients with Coronary Heart Disease

摘要 目的:比较非体外循环冠状动脉搭桥术(OPCABG)与体外循环冠状动脉搭桥术(CCABG)治疗冠心病(CHD)患者的临床疗效。方法:选取2018年2月~2023年11月本院收治的86例CHD患者,根据随机数字表法分为观察组和对照组各43例。观察组和对照组分别采用OPCABG和CCABG治疗。比较两组临床疗效。结果:观察组手术时间、术中出血量、术后呼吸机辅助时间、24h引流量、住院时间均低于对照组(P<0.05);术后24h,两组血清肌钙蛋白I(cTnI)、脑钠肽(BNP)、肌酸激酶同工酶(CK-MB)均较治疗前升高,但观察组明显低于对照组(P<0.05);观察组并发症发生率为11.63%,明显低于对照组的37.21%(P<0.05)。结论:与CCABG比较,OPCABG治疗CHD临床疗效更佳,该术式能有效减轻患者心肌损伤,缩短手术时间和减少出血量,降低不良反应发生率,有利于术后康复。 Objective:To compare the clinical efficacy of off-pump coronary artery bypass grafting(OPCABG)and extracorporeal coronary artery bypass grafting(CCABG)in patients with coronary heart disease(CHD).Methods:86 CHD patients admitted to our hospital from February 2018 to November 2023 were randomly divided into an observation group and a control group,with 43 cases in each group,using a random number table method.The observation group and control group were treated with OPCABG and CCABG,respectively.Compare the clinical efficacy of two groups.Results:The observation group had lower surgical time,intraoperative blood loss,postoperative ventilator assistance time,24-hour drainage volume,and hospitalization time than the control group(P<0.05);24 hours after surgery,serum levels of cardiac troponin I(cTnI),brain natriuretic peptide(BNP),and creatine kinase isoenzyme(CK-MB)increased in both groups compared to before treatment,but the observation group was significantly lower than the control group(P<0.05);The incidence of complications in the observation group was 11.63%,significantly lower than the 37.21%in the control group(P<0.05).Conclusion:Compared with CCABG,OPCABG has better clinical efficacy in the treatment of CHD,which can effectively reduce the myocardial injury of patients,shorten the operation time and reduce the amount of blood loss,and reduce the incidence of adverse reactions,which is conducive to postoperative rehabilitation of patients.
