摘要
目的探讨基于量化评估策略的呼吸训练干预在老年肺癌根治术患者中的应用效果。方法根据干预方式的不同将80例老年肺癌根治术患者分为对照组(n=39)和呼吸训练组(n=41),对照组患者接受常规干预,呼吸训练组患者接受基于量化评估策略的呼吸训练干预。比较两组患者的肺功能指标[用力肺活量(FVC)、第一秒用力呼气容积(FEV_(1))]、血气分析指标[动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))]、术后恢复指标和并发症发生情况。结果干预后,两组患者FVC、FEV_(1)均高于本组干预前,呼吸训练组患者FVC、FEV_(1)均高于对照组,差异均有统计学意义(P﹤0.05)。干预后,两组患者PaO_(2)、SaO_(2)均高于本组干预前,呼吸训练组患者PaO_(2)、SaO_(2)均高于对照组,差异均有统计学意义(P﹤0.05)。呼吸训练组患者的术后置管时间、术后呼吸机使用时间、住院时间均短于对照组,并发症总发生率为4.88%,低于对照组患者的20.51%,差异均有统计学意义(P﹤0.05)。结论基于量化评估策略的呼吸训练干预能够明显改善老年肺癌根治术患者的肺功能指标和血气分析指标,缩短患者的术后恢复时间,降低并发症发生率。
Objective To explore the application effect of respiratory training intervention based on quantitative evaluation strategy in elderly lung cancer patients undergoing radical resection.Method According to different intervention methods,80 elderly lung cancer patients undergoing radical resection were divided into control group(n=39)and respiratory training group(n=41).Patients in the control group received routine intervention,while patients in the respiratory training group received respiratory training intervention based on quantitative evaluation strategy.The lung function indicators[forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1))],blood gas analysis indicators[arterial partial pressure of oxygen(PaO_(2)),arterial blood oxygen saturation(SaO_(2))],postoperative recovery indicators,and incidence of complications were compared between the two groups.Result After intervention,the FVC and FEV_(1)in both groups were higher than those before intervention,the FVC and FEV_(1)in respiratory training group were higher than those in control group,and the differences were statistically significant(P<0.05).After intervention,the PaO_(2)and SaO_(2)in both groups were higher than those before intervention,the PaO_(2)and SaO_(2)in respiratory training group were higher than those in control group,and the differences were statistically significant(P<0.05).The postoperative intubation time,postoperative ventilator use time,and hospitalization time in respiratory training group were shorter than those in control group,the total incidence of complications in respiratory training group was 4.88%,which was lower than 20.51%in control group,and the differences were statistically significant(P<0.05).Conclusion The respiratory training intervention based on quantitative evaluation strategy can significantly improve lung function indicators and blood gas analysis indicators in elderly lung cancer patients undergoing radical resections,shorten the postoperative recovery time,and reduce the incidence of complications.
