基于自噬探讨活血化痰饮治疗激素性股骨头坏死作用机制

Exploring the mechanism of Huoxue Huatan Yin on treating steroid-induced avascular necrosis of the femoral head based on autophagy

摘要 目的基于自噬探讨活血化痰饮对激素性股骨头坏死的影响及作用机制。方法选取30只雄性新西兰大白兔随机分为空白对照组(n=6)与造模组(n=24),采用罗天有改良激素联合脂多糖方法建立兔激素性股骨头坏死模型,将造模后的实验兔随机分为模型组,低、中、高剂量中药组。空白对照组及模型组给予生理盐水灌胃,低、中、高剂量中药组分别给予不同剂量中药灌胃,连续给药8周。干预结束后手术取出双侧股骨头,通过HE染色、光镜观察进行组织形态学及空骨陷窝率比较,蛋白印迹法检测自噬相关蛋白LC3Ⅱ、Beclin-1蛋白表达。结果HE染色结果显示,与空白对照组比较,模型组骨小梁结构紊乱,部分骨小梁变薄甚至断裂,脂肪细胞明显增多,各中药组骨小梁损伤情况均有不同程度改善,造血细胞及髓内组织丰富。空骨陷窝率结果显示,模型组及中药各组空骨陷窝率均高于空白对照组(P<0.05),高剂量组空骨陷窝率下降明显。Western blot结果显示,造模后自噬相关蛋白LC3Ⅱ、Beclin-1表达明显增强,模型组及各中药组LC3Ⅱ、Beclin-1表达均明显高于空白对照组(P<0.05);中、高剂量中药组LC3Ⅱ、Beclin-1表达较模型组下降(P<0.05)。结论活血化痰饮能够改善激素性股骨头坏死模型兔骨坏死表现,其机制可能是通过抑制自噬的过度表达发挥治疗激素性股骨头坏死的作用。 Objective To explore the effects and mechanism of Huoxue Huatan Yin on steroid-induced avascular necrosis of the femoral head(SANFH)based on autophagy.Methods Thirty male New Zealand white rabbits were randomly divided into a blank control group(n=6)and a modeling group(n=24).A rabbit model of SANFH was established using Luo Tianyou’s improved hormone combined with lipopolysaccharide.After the successful modeling was detected by MRI,the experimental rabbits were randomly divided into a model group,and low-,medium-,and high-dose traditional Chinese medicine groups.The blank control group and model group were given normal saline by gavage,while the low-,medium-,and high-dose traditional Chinese medicine groups were given different doses of traditional Chinese medicine by gavage,all for 8 consecutive weeks.The general conditions of the mental state,water and diet of the experimental rabbits were observed in each group.After the intervention,bilateral femoral heads were surgically removed,and their histological morphology and empty bone pit rates were compared through HE staining and light microscopy.Western blot was used to detect the protein expression of autophagy related proteins LC3II and Beclin-1.Relevant data were recorded and statistically analyzed using the SPSS 25.0 software.Results HE staining results showed that the blank control group had an intact bone trabecular structure,abundant bone marrow content,and a large number of hematopoietic cells visible in the medullary cavity,while the structure of bone trabeculae in the model group was disordered,with some bone trabeculae thinning or even broken,and a significant increase in adipocytes,and each traditional Chinese medicine group showed varying degrees of improvement in bone trabecular injury compared to the model group,with more abundant hematopoietic cells and intramedullary tissue.The results showed that the blank control group had only a small amount of empty bone pits in the femoral head tissue,and the rates of empty bone pits were higher in the model and traditional Chinese medicine groups than those in the blank control group(P<0.05).The empty bone pit rate of each traditional Chinese medicine group decreased significantly compared to that in the model group(P<0.05).Among them,the high-dose group showed the most significant decrease.Western blot results showed that the expression of autophagy related proteins LC3II and Beclin-1 was significantly enhanced after modeling,and the expression of autophagy related proteins in the model group and traditional Chinese medicine group was significantly increased compared to that in the blank control group(P<0.05),while the protein expression in the medium-and high-dose traditional Chinese medicine groups was significantly decreased compared to that in the model group(P<0.05).The protein expression in the low-dose group showed no significant difference compared to that in the model group.Conclusion Huoxue Huatan Yin can improve the expression of bone necrosis in SANFH model rabbits,and its mechanism maybe attributes to inhibiting the over-expression of autophagy to treat the disease.
