摘要
目的:探讨在老年股骨颈骨折患者康复中采取快速康复外科理念(ERAS)护理的应用效果。方法:选择2019年1月—2023年12月永丰县人民医院收治的60例老年股骨颈骨折患者为研究对象,根据随机数字表法将其分为观察组和对照组,各30例。对照组采取常规护理干预,观察组采取ERAS护理干预。两组均观察随访2个月。比较两组疼痛程度、骨折愈合时间、功能康复情况、并发症发生情况及生活质量。结果:术后3、6 h,与对照组比较,观察组数字等级评定量表评分均较低,差异均有统计学意义(P<0.05);观察组骨折愈合时间短于对照组,差异有统计学意义(P<0.05)。干预2个月后,两组Harris髋关节功能评分量表各项评分均高于干预前,且观察组均高于对照组,差异均有统计学意义(P<0.05)。观察组并发症发生率6.67%低于对照组的26.68%,差异有统计学意义(P<0.05)。干预2个月后,两组生活质量综合评定问卷-74各项评分均高于干预前,且观察组均高于对照组,差异均有统计学意义(P<0.05)。结论:ERAS护理不仅能够缓解老年股骨颈骨折患者的疼痛程度,缩短其骨折愈合时间,促进功能康复,还能够减少并发症发生,改善患者生活质量。
Objective:To investigate the application of enhanced recovery after surgery(ERAS)nursing in the rehabilitation of elderly patients with femoral neck fracture.Methods:A total of 60 elderly patients with femoral neck fracture admitted to Yongfeng County People’s Hospital from January 2019 to December 2023 were selected as research objects,and were divided into an observation group and a control group according to random number table method,with 30 cases in each group.The control group received conventional nursing intervention,and the observation group received ERAS nursing intervention.Both groups were observed and followed up for 2 months.The pain degree,fracture healing time,functional rehabilitation,complications and quality of life were compared between the two groups.Results:After 3,6 h of operation,compared with the control group,the numerical rating scale scores of the observation group were lower,the differences were statistically significant(P<0.05);the fracture healing time of observation group was shorter than that of control group,and the difference was statistically significant(P<0.05).After 2 months of intervention,the scores of Harris Hip Function Scale in both groups were higher than before intervention,and the observation group was higher than the control group,with statistical significance(P<0.05).The incidence of complications in the observation group was 6.67%lower than 26.68%in the control group,the difference was statistically significant(P<0.05).After 2 months of intervention,the scores of Generic Quality of Life Inventory-74 in both groups were higher than before intervention,and the observation group was higher than the control group,with statistical significance(P<0.05).Conclusion:ERAS nursing can not only alleviate the pain degree of elderly patients with femoral neck fracture,shorten the healing time of fracture,promote functional rehabilitation,but also reduce the occurrence of complications and improve the quality of life of patients.
