摘要
多囊卵巢综合征(PCOS)是妇科常见的内分泌异常与生殖功能受损并存的疾病,高雄激素血症是其核心诊断标准之一,临床常以游离睾酮值或总睾酮值判断该病高雄激素的程度。中医认为该病的发生与肾、脾、肝、冲任二脉及胞宫有关,多由肾虚夹瘀、肝失疏泄、脾虚夹痰所致的瘀血和痰湿而引发。近年来中西医结合方法降低PCOS患者血清睾酮水平的临床研究日益增多,同时可改善患者内分泌紊乱、生殖功能异常、月经周期异常等临床表现,效果显著。本文综述PCOS高雄激素血症发生的中医病因病机、PCOS患者高睾酮值西医病理机制及中西医结合降低PCOS患者高血睾水平的临床方法和效果进展。
Polycystic ovary syndrome(PCOS)is a common gynecological disease characterized by endocrine abnormalities and impaired reproductive function.Hyperandrogenism is one of its core diagnostic criteria,and the degree of hyperandrogenism in the disease is often judged by free testosterone or total testosterone levels in clinical practice.Traditional Chinese medicine believes that the occurrence of this disease is related to kidney,spleen,liver,thoroughfare channel,conception channel and uterus.It is often caused by kidney deficiency with blood stasis,liver un-controlling conveyance and dispersion,spleen deficiency with phlegm,resulting in blood stasis and phlegm dampness.In recent years,there has been an increasing number of clinical studies on decreasing serum testosterone level in patients with PCOS by integration of traditional and western medicine method,at the same time,it can improve clinical manifestations such as endocrine disorder,reproductive dysfunction,menstrual cycle abnormalities,with significant effect.This article reviews traditional Chinese medicine etiology and pathogenesis of hyperandrogenism in PCOS,western medicine pathological mechanism of high testosterone levels in patients with PCOS,clinical method and effect progress on decreasing high testosterone level in patients with PCOS by integration of traditional and western medicine.
