牙周引导组织再生术治疗下颌阻生第三磨牙拔除后第二磨牙远中深牙周袋疗效评估

Evaluation of Periodontal-guided tissue regeneration for the treatment of deep periodontal pockets on the distal surface of mandibular second molars after extraction of impacted mandibular third molar

摘要 目的:探讨牙周引导组织再生技术对下颌第二磨牙远中深牙周袋疗效评估。方法:对2021年7月至2023年7月于东莞健力口腔医院牙周科收治的15位患者20个远中面存在骨下缺损≥3 mm,曾有阻生第三磨牙拔除病史的下颌第二磨牙,先行牙周基础治疗后6周评估,探诊深度(PD)≥6 mm,并伴探诊出血,行牙周引导组织再生术,联合使用Bio-Oss骨粉及可吸收胶原膜。术前、术后6个月、12个月,检查临床指标,PD,龈沟出血指数(SBI),松动度(TM),临床附着丧失(CAL),X线片检查骨缺损充填情况,并进行统计分析。结果:术后6、12个月牙周袋PD较术前均显著减小,SBI明显下降,TM有所改善,CAL明显降低,临床附着恢复理想,根尖片显示骨缺损可见充足骨充填影像,有统计学意义(P<0.05),术后12个月与术后6个月各项指标对比,差异无统计学意义(P>0.05)。结论:牙周引导组织再生术对下颌阻生第三磨牙拔除后第二磨牙远中深牙周袋可取得良好疗效。 AIM:To investigate the efficacy of periodontal guided tissue regeneration technology on deep periodontal pockets of distal mandibular second molars.METHODS:15 patients with 20 mandibular second molars subbony defects≥3 mm in the distal surface and had a history of impacted third molar extraction admitted in the Department of Periodontology of Dongguan Jianli Stomatological Hospital,during July 2021 to July 2023.6 weeks after the periodontal basic treatment,found probing depth(PD)≥6 mm,and probing bleeding,then periodontal guided tissue regeneration was performed.Bio-Oss bone powder is used in combination with absorbable collagen membranes.Before surgery,6 months and 12 months after surgery,clinical indicators included PD and after surgery,clinical indicators,PD,gingival sulcus bleeding index(SBI),tooth mobility(TM),clinic attachment loss(CAL),periapical radiograph were examined.And statistical analysis was performed.RESULTS:Compared with the preoperative periodontal pocket PD,gingival sulcus SBI decreased significantly,TM was improved,the clinical CAL was significantly reduced,and the clinical attachment recovery was satisfactory,and periapical radiograph showed that the bone defect with sufficient bone filling images,which was statistically significant(P<0.05),and the difference between the indexes at 12 months after surgery and 6 months after surgery was not statistically significant(P>0.05).CONCLUSION:Periodontal guided tissue regeneration can achieve good results in the distal deep periodontal pockets of mandibular second molars after extraction of impacted third molar.
