摘要
目的 探讨滋肾育胎丸治疗高龄先兆流产(TA)孕妇的临床效果及其对性激素水平和妊娠结局的影响。方法 将2020年6月~2023年6月收治的114例高龄TA孕妇,随机分成对照组和研究组(各57例)。对照组患者采用常规西药治疗,研究组患者在对照组基础上加用滋肾育胎丸治疗。比较两组患者中医症候积分、临床疗效、性激素[血清孕酮(P)、人绒毛膜促性腺激素(HCG)、黄体生成激素(LH)、雌二醇(E_(2))]水平、妊娠结局及不良反应发生情况。结果 治疗14 d后,两组患者中医证候总积分及主症、次症积分较治疗前降低,且研究组低于对照组(P<0.01);研究组患者临床疗效总有效率高于对照组(P<0.05);两组患者血清P、HCG、LH、E_(2)水平较治疗前升高,且研究组高于对照组(P<0.05或0.01)。研究组患者保胎成功率及新生儿Apgar评分高于对照组(P<0.01),而两组患者不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 滋肾育胎丸治疗高龄TA孕妇疗效确切,能够促进性激素分泌,改善妊娠结局,具有一定安全性。
Objective To explore the clinical effect of zishen yutai pill in elderly pregnant women with threatened abortion(TA)and its influence on sex hormone level and pregnancy outcome.Methods A total of 114 elderly TA pregnant women hospitalized from Jun.2020 to Jun.2023 were randomly divided into control and research group(with 57 ones in each).Controls received conventional Western medicine treatment and research group was plus zishen yutai pill on the basis of controls.Such indexes were compared between two groups as TCM syndrome scores,clinical effect,sex hormones(serum progesterone[P],human chorionic gonadotropin[HCG],luteinizing hormone[LH],estradiol[E_(2)])levels,pregnancy outcomes,and adverse reactions.Results After 14 day treatment,the total,main and secondary syndrome scores of TCM syndromes lowered in both groups compared with pre-treatment and those were lower in research than control group(P<0.01),clinically total effective rate was higher in research than control group(P<0.05),serum P,HCG,LH and E_(2) levels elevated in both groups compared with pre-treatment and those were higher in research than control group(P<0.05 or 0.01).The success rate of fetal protection and Apgar score of newborns were higher in research than control group(P<0.01),intergroup difference in the overall incidence of adverse reactions was not statistically significant(P>0.05).Conclusion Zishen yutai pill is effective in the treatment of elderly TA pregnant women,can promote sex hormone secretion,improve pregnancy outcome,and has a certain safety.
