摘要
突发性耳聋(简称突聋)即中医所指的耳聋,在西医上属于特发性耳聋,是耳鼻喉科常见的疾病之一,具有发病率高、病情急、进展迅速、发病人群逐渐年轻化等特点。听觉对每个人来说都至关重要,无论是工作还是生活,都需要有良好的听觉辅助我们,突发听觉丧失或听力减退会对患者的工作和生活等造成严重的影响。针对突聋,现代研究表明微循环障碍是其主要病因,改善微循环为治疗突聋的一大重要手段。治病必求于本,从根本论治是中医治病的关键。通窍活血汤在中医上主要的功效为行气活血、化瘀通络,现代药理学也表明其发挥着抗炎、抗血小板聚集、保护细胞等作用,对于改善血流动力学作用显著;石菖蒲和煅磁石在中医上重在聪耳窍,同时具备抗炎杀菌、提高免疫力等现代药理作用。仝小林院士的态靶理论是连接宏观与微观的桥梁,其态有“状态、动态、态势”三层含义,调态即从宏观入手,运用药物的偏性促进人体阴阳平衡;靶分为“病靶、症靶、标靶”,打靶即从微观角度出发,提高临床治疗的靶向性和精准性。受仝小林院士态靶结合理论的影响,当以瘀为态,耳聋为症靶,采用通窍活血汤加石菖蒲、煅磁石治疗突聋,该组合具有多靶点、多通路、不良反应小等特点,患者接受度高,临床疗效显著。
Sudden deafness referrs to deafness in traditional Chinese medicine,which belongs to idiopathic deafness in Western medicine,and it is one of the common diseases in otolaryngology,with the characteristics of high incidence,acute attack,rapid progress,and gradually younger patients,etc.Hearing is crucial for everyone,both at work or life,we need to have a good hearing to assist us,and sudden hearing loss or disability of hearing will have a serious impact on the work and life of patients.For sudden deafness,modern research shows that microcirculation disorder is the main cause of sudden deafness,and improving microcirculation has become an important means to treat sudden deafness.The treatment must target the root causes,and treating from the root is the key of TCM treatment.The main functions of Tongqiao Huoxue decoction(通窍活血汤)in traditional Chinese medicine are activating Qi(气)and blood circulation,removing blood stasis and clearing collaterals.Modern pharmacology also shows that Tongqiao Huoxue decoction plays a significant role in improving hemodynamics,such as anti-inflammatory,anti-platelet aggregation and cell protection.In traditional Chinese medicine,Shichangpu(Rhizoma Acori Tatarinowii)and Duan Cishi(Calcined Magnetitum)focus on improving the hearing,and have modern pharmacological functions such as anti-inflammation,bactericidal efficacy and immune enhancement.Academician TONG Xiaolin’s theory of state and target is a bridge between macro and micro,and its‘state’has three meanings of‘state,dynamic state and situation’.Regulating state is to use the bias of drugs to promote the balance of Yin(阴)and Yang(阳)in the human body from a macro perspective.The target is divided into‘disease target,symptom target and goal target’,which is to improve the targeting and accuracy of clinical treatment from a microscopic point of view.Inspired by Academician TONG Xiaolin’s theory of the combination of state and target,Tongqiao Huoxue decoction plus Shi Changpu and Duan Cishi are used to treat sudden deafness with blood stasis as the state and deafness as the target.The combination has the characteristics of multiple targets,multiple pathways and few adverse reactions,with high patients’acceptance and significant clinical effects.
