摘要
目的:探讨中药治疗异位妊娠(Ectopic Pregnancy,EP)的用药规律,为中医药治疗EP提供参考。方法:计算机检索中国知网数据库中运用中医药治疗EP的临床研究文献,拟定数据纳入排除标准,收集临床治疗EP的中药处方,利用Microsoft Excel 2019建立处方数据库。借助中医传承辅助平台V2.5,基于频数统计分析用药频次、四气五味和归经分布,基于关联规则的组方规律提取出常用药物组合,分析药对用药规则。基于改进的互信息法和复杂系统熵聚类,设定相关度,提取出核心药物组合,并通过无监督的熵层次聚类算法进一步演化得到新方组合。结果:共纳入符合标准的文献608篇,收集537首中药方剂。统计发现方剂的药味组成数以8~11味为主,单纯应用中医适宜技术的研究文献较少,绝大多数临床研究联用了甲氨蝶呤或米非司酮;涉及中药共175味,其中使用频数≥50的高频药共18味,以赤芍、丹参、桃仁、莪术、三棱、天花粉最常用,药性以寒、温、平居多,五味以苦、辛、甘为主,主要归肝经、心经、脾经;基于关联规则分析得到常用药物组合28条,以“赤芍-丹参”最常见;基于系统聚类分析挖掘出4个新方组合。结论:临床保守治疗EP的用药方案以中药联用甲氨蝶呤和米非司酮为主,以活血祛瘀为治疗大法,兼顾散肿止痛、补血益气。
Objective:To explore the medication rules of Chinese medicine in the treatment of ectopic pregnancy to provide reference for the treatment of ectopic pregnancy with Chinese medicine.Methods:The clinical research literature on the treatment of ectopic pregnancy with traditional Chinese medicine in CNKI database was searched by computer,data inclusion and exclusion criteria were developed,Chinese medicine prescriptions for the clinical treatment of ectopic pregnancy were collected,and a prescription database was es-tablished using Microsoft Excel 2019.The frequency of medication,four properties,five flavors and meridian tropism of the medicinals were analyzed based on frequency statistics by TCMISS V2.5,an auxiliary platform for TCM inheritance.Based on the rules of association rules,the commonly used drug combinations were extracted,and the rules of drug pairs were analyzed.Based on the improved mutual information method and entropy clustering of complex system,the correlation degree was set,the core drug combination was extracted,and the new formula combina-tion was further evolved by unsupervised entropy hierarchical clustering algorithm.Results:A total of 608 studies met the criteria were included,and 537 TCM prescriptions were collected.It was found that the composition of the prescription was mainly 8 to 11 medicine.There were a few studies on the application of traditional Chinese medicine,and the vast majority of clinical studies used methotrexate or mifepristone.A total of 175 TCM medicine were involved,among which 18 high-frequency medicines with a frequency of≥50 were used.Chishao(Radix paeoniae rubra),Danshen(Salvia miltiorrhiza),Taoren(peach kernel),Ezhu(Rhizoma curcumae),Sanleng(Rhizoma sparganii)and Tianhua fen(Radix trichosanthis)were the most commonly used.The drug properties were cold,warm and moderate mainly,and the five flavors were bitter,spicy and sweet,which mainly flowed into liver,heart and spleen meridians.Based on association rule analysis,28 commonly used drug combinations were obtained,and Chishao-Danshen was the most common.Based on systematic clustering analysis,four new combinations of prescriptions were excavated.Conclusion:Clinical conservative treatment of ectopic pregnancy is mainly Chinese medicine combined with methotrexate and mifepristone,traditional Chinese medicine treatment principle is to promote blood circulation and remove stasis,taking into account the removal of swelling and relieving pain,and invigorating Qi(气)and blood.
