摘要
目的探讨自拟促孕汤联合枸橼酸氯米芬治疗对肾虚型排卵障碍性不孕(ODI)患者性激素水平及生殖系统功能的影响。方法选取2021年7月—2023年6月广东省中西医结合医院收治的60例肾虚型ODI患者为研究对象,按随机数字表法将其分为对照组和观察组,每组30例。对照组采用枸橼酸氯米芬治疗,观察组在对照组的基础上联合自拟促孕汤治疗,两组均持续治疗3个月经周期。比较两组的性激素水平、中医证候积分、生殖系统的功能、排卵情况、妊娠情况、不良反应发生情况。结果治疗3个月经周期后的月经周期第14天,观察组雌二醇、促黄体生成素水平均高于对照组,组间差异有统计学意义(P<0.05);两组促卵泡生成素水平比较,差异无统计学意义(P>0.05)。治疗3个月经周期后,观察组月经量、经期、经色、腰骶酸痛、性欲减退评分均低于对照组,组间差异有统计学意义(P<0.05)。治疗3个月经周期后的月经周期第12天,观察组子宫内膜厚度厚于对照组,优势卵泡直径均大于对照组,子宫动脉阻力指数、搏动指数均低于对照组,组间差异有统计学意义(P<0.05)。观察组排卵、妊娠率分别为90.00%、50.00%,均高于对照组的60.00%、20.00%,组间差异有统计学意义(P<0.05)。两组均未见明显不良反应发生。结论相比于枸橼酸氯米芬单一治疗,联合自拟促孕汤治疗肾虚型ODI患者效果更佳,且安全可靠,值得临床推广应用。
Objective Explore the impact of self-formulated pregnancy-promoting decoction combined with clomiphene citrate on the sex hormone levels and reproductive system function of patients with kidney deficiency type ovulatory dysfunction infertility(ODI).Methods Sixty patients with kidney deficiency type ODI admitted to Guangdong Hospital of Integrated Traditional Chinese and Western Medicine from July 2021 to June 2023 were selected as the research subjects.They were divided into a control group and an observation group according to the random number table method,with 30 patients in each group.The control group was treated with clomiphene citrate,and the observation group was treated with self-formulated pregnancy-promoting decoction in combination with the treatment of the control group,both groups were continuously treated for three menstrual cycles.The sex hormone levels,Traditional Chinese Medicine syndrome scores,reproductive system functions,ovulation status,pregnancy status,and occurrence of adverse reactions were compared between the two groups.Results On the 14th day of the menstrual cycle after three menstrual cycles of treatment,the levels of estradiol,and luteinizing hormone in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).There was no significant difference in follicle-stimulating hormone levels between the two groups(P>0.05).After three menstrual cycles of treatment,the scores of menstrual volume,menstrual period,menstrual color,lumbosacral soreness,and decreased libido in the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).On the 12th day of the menstrual cycle after three menstrual cycles of treatment,the endometrial thickness in the observation group was thicker than that in the control group,the diameter of dominant follicles was larger than that in the control group,and uterine artery resistance index and pulobation index were lower than those in the control group,with differences between the groups were statistically significant(P<0.05).The ovulation rate and pregnancy rate in the observation group were 90.00%and 50.00%,respectively,both of which were higher than 60.00%and 20.00%in the control group,and the differences between the groups were statistically significant(P<0.05).No obvious adverse reactions occurred in either group.Conclusion Compared with the single treatment of clomiphene citrate,the combined treatment of self-formulated pregnancy-promoting decoction and clomiphene citrate has a better effect on patients with kidney deficiency type ODI.It is safe and reliable,and worthy of clinical promotion and application.
