摘要
目的:观察柴胡疏肝散加减联合体外反搏对冠心病经皮冠状动脉介入术(PCI)术后患者焦虑抑郁状态和心肺功能的影响。方法:按随机数字表法将120例PCI术后焦虑抑郁状态和心肺功能异常的患者分为对照组、中药组、体外反搏组、联合组,每组30例。对照组予常规西药治疗,中药组在对照组基础上加用柴胡疏肝散加减,体外反搏组在对照组基础上加用体外反搏治疗,联合组在对照组基础上加用柴胡疏肝散加减联合体外反搏治疗。观察四组治疗前后用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、最高运动状态时代谢当量(Peak METs)、峰值摄氧量(Peak VO_(2))、每公斤峰值摄氧量(Peak VO_(2)/kg)、峰值氧脉(Peak VO_(2)/HR)、二氧化碳通气当量斜率(VE/VCO_(2)-SLOPE)、17项汉密尔顿抑郁量表(HAMD)、14项汉密尔顿焦虑量表(HAMA)、中医证候积分、临床疗效。结果:治疗后四组患者心肺运动指标较治疗前均有改善(P<0.05)。治疗后,联合组METs值高于对照组、中药组、体外反搏组(均P<0.05)。联合组FVC、FEV1、Peak O_(2)/HR、Peak VO_(2)、Peak VO_(2)/kg值高于对照组、中药组(均P<0.05)。治疗后,四组HAMA、HAMD评分下降,联合组HAMA、HAMD评分下降程度高于对照组、中药组、体外反搏组(均P<0.05)。观察组中医证候疗效的总有效率96.7%,高于中药组、体外反搏组、对照组。结论:柴胡疏肝散加减联合体外反搏可显著提高PCI术后心肺功能,改善患者焦虑抑郁状态。
Objective:To observe effect of Chaihu Shugan powder combined with external counter pulsation on anxiety depression and cardiopulmonary function in patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods:According to random number table method,120 patients with anxiety depression and cardiopulmonary function after PCI were divided into control group,Chinese medicine group,external counter pulsation group and combined group,30 cases in each group.The control group were treated with conventional western medicine,the Chinese medicine group treated with Chaihu Shugan powder on the basis of the control group,the external counter pulsation group treated with external counter pulsation on the basis of the control group,and the combined group were treated with Chaihu Shugan powder combined with external counter pulsation on the basis of the control group.The forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),metabolic equivalents at the highest exercise state(Peak METs),peak oxygen uptake(Peak VO_(2)),peak oxygen uptake per kilogram(Peak VO_(2)/kg),peak oxygen pulse(Peak VO_(2)/HR),carbon dioxide ventilation equivalent slope(VE/VCO_(2)-SLOPE),17 Hamilton depression scale(HAMD)and 14 Hamilton anxiety scale(HAMA)were observed before and after treatment in the four groups.TCM syndrome scores and TCM syndrome efficacy of the four groups were observed before and after treatment.Results:After treatment,the cardiopulmonary exercise indexes of the four groups were improved,compared with those before treatment(P<0.05).The METs value of the combined group were higher than that in the control group,the Chinese medicine group and the external counter pulsation group(all P<0.05).The values of FVC,FEV1,Peak O_(2)/HR,Peak VO_(2),and peak VO_(2)/kg in the combined group were higher than those in the control group and the Chinese medicine group(all P<0.05).The HAMA and HAMD scores of the four groups decreased(all P<0.05),and the HAMA and HAMD scores of the combined group decreased better than those in the control group,the Chinese medicine group and the external counter pulsation group(all P<0.05).The total effective rate of TCM syndromes in the observation group was 96.7%higher than that in the traditional Chinese medicine group,the external counter pulsation group and the control group.Conclusion:Chaihu Shugan powder combined with external counter pulsation intervention can significantly improve cardiopulmonary function of patients after PCI and improve anxiety and depression of patients after PCI.
