摘要
总结江苏省名中医王培民教授审证求因,治病求本,探讨寒湿对膝骨关节炎(knee osteoarthritis,KOA)的病因病机、致病特点,并应用膝痹宁Ⅱ结合中医外治综合治疗的学术思想及临床经验。KOA属于“痹证”范畴,证属本虚标实,本虚主要指患者年老,肝肾亏虚,不能濡养筋骨,标实指风、寒、湿、热等外邪侵袭是KOA的致病因素。寒湿外邪为KOA首要病因,针对临床最常见的寒湿痹阻型骨关节炎,王培民教授总以温阳通络之法,在温补肾阳同时,着重祛寒利湿,舒筋止痛,结合易层贴膏、泡脚方等中医外治,效果满意。
Professor WANG Peimin,a renowned traditional Chinese medicine(TCM)practitioner from Jiangsu Province,examines the etiology and pathogenesis of knee osteoarthritis(KOA),focusing on the role of cold-dampness as a primary cause.He explores the characteristics of the disease and applies the academic thought and clinical experience of integrated treatment with external TCM therapies,including the use of“Xibi NingⅡ”(a proprietary Chinese medicine).KOA falls under the category of“Bi syndrome,”characterized by a deficiency in the root(essence)and an excess in the branches(symptoms).The root deficiency mainly refers to the aging of patients,with liver and kidney deficiency leading to insufficient nourishment of the tendons and bones.The excess symptoms are due to external pathogenic factors such as wind,cold,dampness,and heat.Cold-dampness is identified as the primary cause of KOA.For the most common clinical type of KOA characterized by cold-dampness obstruction,Professor WANG consistently employs methods to warm Yang and unblock channels.While tonifying the kidney Yang,he emphasizes the removal of cold and dampness,relaxing tendons,relieving pain,and combines this with external treatments such as the application of multi-layered plasters and foot soaking formulas,achieving satisfactory results.
