摘要
目的研究桑白皮清肺化痰方辅助治疗咳嗽变异性哮喘(cough variant asthma,CVA)对患儿疗效的影响。方法将2022年2月至2024年2月在郑州人民医院接受治疗的80例CVA患儿选为观察对象,用数字随机表法分为对照组和观察组各40例。对照组给予布地奈德联合孟鲁司特钠咀嚼片治疗,观察组在对照组的基础上给予桑白皮清肺化痰方辅助治疗,连续治疗7 d。比较两组CVA患儿在治疗前后的中医症候积分、痰液中EOS及炎性因子[白细胞介素-5(IL-5)、白细胞介素-4(IL-4)]水平、肺功能[第1秒用力呼气容积(forced expiratory volume at 1 second,FEV_(1))、用力肺活量(forced vital capacity,FVC)、呼气流量峰值(peak expiratory flow,PEF)],观察治疗过程中不良反应发生情况。结果治疗后,CVA患儿的中医症候积分、痰液中EOS、IL-5、IL-4均明显降低(P<0.05),提示观察组症状减轻及炎症消退的效果更佳;与对照组相比,CVA患儿的FEV_(1)、FVC、PEF均明显升高(P<0.05),提示观察组肺功能改善效果更佳;两组CVA患儿的不良反应总发生率无明显性差异(P>0.05),提示两组所用疗法安全性均属良好。结论桑白皮清肺化痰方辅助治疗CVA患儿能有效缓解临床症状,改善肺功能,降低痰液中EOS与炎性因子水平,安全性良好。
Objective To study the effect of mulberry bark qingfeihuatan formula on the level of eosinophils(EOS)in the sputum of children with cough variant asthma(CVA).Methods A total of 80 children with CVA treated in Zhengzhou People′s Hospital from February 2022 to February 2024 were selected as observation objects,and were randomly divided into observation group and control group,with 40 cases in each group by digital random table method.The control group was given budesonide combined with montelukast sodium chewable tablets,and the observation group was given mulberry bark qingfeihuatan formula on the basis of the control group for continuous treatment for 7 days.The traditional Chinese medicine symptom scores,EOS in sputum and inflammatory factors[interleukin-5(IL-5),interleukin-4(IL-4)]levels,and lung function[forced expiratory volume in 1 second(FEV_(1)),forced vital capacity(FVC),peak expiratory flow(PEF)]of the two groups of children with CVA before and after treatment were compared,and the occurrence of adverse reactions during the treatment process was observed.Results After treatment,the TCM symptom score,EOS,IL-5 and IL-4 in sputum of CVA children in both groups were significantly decreased(P<0.05),and the observation group was lower than the control group(P<0.05),suggesting that the observation group had better effect of symptom relief and inflammation resolution;FEV_(1),FVC and PEF of CVA children in both groups were significantly increased(P<0.05),and the observation group was higher than the control group(P<0.05),suggesting that the observation group had a better effect on lung function;there was no significant difference in the total incidence of adverse reactions between the two groups of CVA children(P>0.05),suggesting that the safety of the treatment used in the two groups was good.Conclusion Mulberry bark qingfeihuatan formula in the treatment of children with CVA can effectively relieve clinical symptoms,improve lung function,reduce the level of EOS and inflammatory factors in sputum and has good safety.
