摘要
目的通过比较2022年1~12月与2023年1~12月外科ICU收治的胃肠道肿瘤高危患者进行分析,探讨AICU的建立对胃肠道肿瘤高危患者术后死亡率的影响。方法本研究为回顾性队列研究。通过查阅住院患者电子病历纳入新疆维吾尔自治区人民医院2022年1~12月与2023年1~12月诊断为胃肠道肿瘤并行根治术的患者453例作为研究对象。根据入住术后ICU的不同将患者分为两组,将2023年1~12月入住AICU的胃肠道肿瘤根治术患者作为试验组(n=239),2022年1~12月入住外科ICU的胃肠道肿瘤根治术患者作为对照组(n=214),对两组患者的临床资料进行分析。结果试验组术后死亡率为7.1%,对照组为12.8%,两组比较差异有统计学意义(P<0.05);试验组患者术后并发症发生率为15.9%,对照组22.4%,两组比较差异有统计学意义(P<0.05);试验组患者术后ICU停留时间为(0.9±1.9)d,对照组患者术后ICU停留时间为(2.1±1.7)d,两组比较差异有统计学意义(P<0.05)。结论通过专业化的术后监护和治疗以及优化围术期管理流程等措施,AICU降低了胃肠道肿瘤根治术患者术后死亡率,提高了治疗效果和安全性,促进了患者的快速康复和生活质量的提高。
Objective To investigate the influence of the establishment of AICU on the postoperative mortality rate of high⁃risk patients with gastrointestinal tumors,by comparing the high⁃risk patients with gastrointestinal tumors admitted to the surgical ICU from January to December 2022 with those in the same period of 2023.Methods This study is a retrospective cohort study.A total of 453 patients diag⁃nosed with gastrointestinal tumors undergoing radical surgery at People's Hospital of Xinjiang Uygur Autonomous Region from January 2022 to December 2022 and from January 2023 to December 2023 were included as the research subjects by reviewing the electronic medical records of inpatients.The patients were divided into two groups based on the different type of postoperative ICU they were admitted to.Altogether 239 patients receiving gastrointestinal tumor radical surgery admitted to AICU from January to December 2023 were allocated to the experimental group(n=239),and 214 patients with gastrointestinal tumor radical surgery admitted to the surgical ICU from January to December 2022 were included to the control group(n=214).The clinical data of the two groups of patients were analyzed.Results Compared with the control group,the postoperative mortality rate of the experimental group was 7.1%,and that of the control group was 12.8%,with a statistically significant difference(P<0.05);the incidence of postoperative complications in the experimental group was 15.9%,which was statistically significantly different from that of the control group(22.4%)(P<0.05);the postoperative ICU stay time of the two groups were 0.9±1.9 days for the experimental group and 2.1±1.7 days for the control group,with a statistically significant difference(P<0.05).Conclusion Through professional postoperative monitoring and treatment,as well as optimizing the perioperative manage⁃ment process,AICU has reduced the postoperative mortality rate of patients undergoing gastrointestinal tumor radical surgery,improved the therapeutic effect and safety,and promoted the rapid recovery and improvement of the life quality of patients.
