摘要
目的通过品管圈(Quality Control Circle,QCC)活动有效提升乳腺X线摄影检查技术的规范率,提高图像质量,加强乳腺癌的早期筛查。方法组建QCC小组,运用QCC手法及科学统计工具如甘特图、柏拉图、鱼骨图等对乳腺X线摄影检查技术中存在的问题进行分析探讨并制定对策。通过比较QCC改善前677例图像和改善后567例图像的影像规范率,评估QCC活动在提升乳腺X线摄影检查技术中的应用效果。结果通过QCC手法及科学统计工具分析得出活动改善重点为:内外斜(Mediolateral Oblique,MLO)位胸大肌未充分显示、双侧头尾(Craniocaudal,CC)位内侧组织无法全部显示、边缘肿块未全部显示。通过改善,乳腺X线摄影检查技术的规范率由51%提升至86%,目标达标成率为109%,其中MLO胸大肌未充分显示、CC内侧组织无法全部显示、边缘肿块未全部显示的问题得到大幅改善,且改善前后差异均有统计学意义(P<0.001)。结论QCC活动在提升乳腺X线摄影检查技术的规范率中效果显著,其形成的标准化作业流程可大幅提高图像质量,对提高乳腺癌早期筛查的精准性起到了积极作用。
Objective To improve the technical specification rate of mammography through the quality control circle(QCC)activities,and to improve the image quality,so as to strengthen the early screening of breast cancer.Methods A QCC team was formed to analyze and discuss the problems existing in mammography techniques and formulate countermeasures by using QCC techniques and scientific statistical tools such as Gantt chart,Berra chart and fishbone chart.By comparing the image standard rate of 677 images before QCC improvement and 567 images after QCC improvement,the application effect of QCC activities in improving mammography was evaluated.Results Through the analysis of QCC techniques and scientific statistical tools,it was concluded that the focus of activity improvement was as follows:insufficient display of pectoralis major muscle in mediolateral oblique(MLO)position,insufficient display of the medial tissue in craniocaudal(CC)position and insufficient display of the marginal mass.Through the improvement,the technical specification rate of mammography was increased from 51%to 86%,and the target compliance rate was 109%.Among them,the problems including insufficient display of pectoralis major muscle in MLO position,insufficient display of the medial tissue in CC position and insufficient display of the marginal mass were significantly improved,and the differences before and after improvement were statistically significant(P<0.001).Conclusion QCC activities have a significant effect on improving the technical specification rate of mammography,and the standardized operation process formed through QCC activities can greatly improve the image quality,which can play a positive role in improving the accuracy of early breast cancer screening.
