成人高血压病人血压季节性变化健康管理的证据总结

Summary of evidence on the health management of seasonal changes in blood pressure in adults with hypertension

摘要 目的检索、提取、评价、整合和分析成人高血压病人血压季节性变化健康管理的相关证据。方法依据“6s”证据金字塔模型,检索相关临床数据库及网站,获取成人高血压病人血压季节性变化健康管理的相关文献。检索的证据类型为证据总结、临床决策、推荐实践、技术报告、临床指南、系统评价和专家共识。检索时限均为建库至2023年3月。由2名研究者独立进行文献筛查、质量评价和证据整合。结果根据纳排标准,共筛选出5篇文献,包括1篇指南,2篇系统评价,2篇专家共识,纳入文献总体质量较高。从亚组人群、变化特征、血压测量、治疗策略和生活方式共5个主题汇总了22条证据。结论血压变化呈现季节性模式,冬季水平较高,而夏季水平较低。根据人群特征和变化特点,规范血压监测,并从治疗策略和生活方式两方面进行健康管理,可以加强高血压病人的疾病防控,改善病人疾病预后。 Objective To retrieve,extract,evaluate,integrate,and analyze evidence related to the health management of seasonal changes in blood pressure in adults with hypertension.Methods Based on the"6s"evidence pyramid model,search relevant clinical databases and websites to obtain relevant literature on health management of seasonal changes in blood pressure in adult hypertensive patients.The types of evidence searched were evidence summaries,clinical decisions,recommended practices,technical reports,guidelines,systematic reviews,and expert consensus.The search time frame for all searches was from the establishment of the database to March 2023.Literature screening,quality evaluation,and evidence integration were performed independently by 2 investigators.Re⁃sults Based on the inclusion and exclusion criteria,5 papers were screened,including 1 guideline,2 systematic reviews,and 2 expert consensus,and the overall quality of the included literature was high.A total of 22 pieces of evidence were pooled from 5 topics,including subgroup population,change characteristics,blood pressure measurement,treatment strategies,and lifestyle.Conclusions Blood pressure changes exhibit a seasonal pattern,with higher levels in winter and lower levels in summer.Standardize blood pressure monitoring based on population characteristics and changes,and carry out health management from both treatment strategies and lifestyle aspects,in order to strengthen disease prevention and control of hypertensive patients and improve their disease prognosis.
