摘要
目的 观察止血祛瘀明目片联合康柏西普眼内注射对糖尿病性白内障术后黄斑水肿(macular edema, DME)的影响。方法 选取该院自2019年4月—2021年12月收治行白内障超声乳化吸除人工晶状体植入术治疗后出现DME的糖尿病患者104例(单眼患病),根据随机数字法分为两组。对照组给予康柏西普眼内注射,而观察组加用止血祛瘀明目片治疗。比较两组最佳矫正视力(best-corrected visual acuity, BCVA)、眼压、黄斑中心凹视网膜厚度(central macular thickness, CMT)、血清相关因子及证候积分的差异。结果 基线时,两组DME患者BCVA、眼压、CMT比较,差异无统计学意义(P>0.05)。治疗1、3个月后,两组DME患者BCVA较基线升高,而眼压、CMT则明显下降,且观察组DME患者BCVA升高和眼压、CMT下降幅度均大于对照组,差异有统计学意义(P<0.05)。基线时,两组DME患者血清相关因子、证候积分比较,差异无统计学意义(P>0.05)。治疗3个月后,两组DME患者干扰素诱导蛋白-10(IP-10)、白细胞介素-6(IL-6)、巨噬细胞趋化蛋白-1(MCP-1)及视物不清、目睛干涩、头晕耳鸣、口咽干燥等证候积分均较基线下降,且观察组DME患者血清相关因子、证候积分下降幅度均大于对照组,差异有统计学意义(P<0.05)。观察组发生皮疹1例,对照组无相关不良反应发生。结论 止血祛瘀明目片联合联合康柏西普眼内注射治疗糖尿病性白内障术后DME可降低眼压,矫正视力,减轻症状和黄斑水肿程度,可能与调节相关血清因子有关。
Objective To observe the effect of Zhixue Quyu Mingmu Tablets(止血祛瘀明目片)combined with intraocular injection of conbercept on macular edema(DME)after diabetic cataract surgery.Methods A total of 104 diabetic patients with DME after phacoemulsification and intraocular lens implantation in the hospital from April 2019 to December 2021 were selected and divided into two groups according to the random number method.The control group was given intraocular injection of conbercept,while the observation group was additionally given Zhixue Quyu Mingmu Tablets.The differences in best-corrected visual acuity(BCVA),intraocular pressure,central macular thickness(CMT),serum-related factors and syndrome scores were compared between the two groups.Results At baseline,there was no significant difference in BCVA,intraocular pressure or CMT between the two groups(P>0.05).After 1 and 3 months of treatment,the BCVA of the DME patients in the two groups increased,while the intraocular pressure and CMT decreased significantly,and the increase of BCVA and the decrease of the intraocular pressure and CMT were more obviously in the observation group than those in the control group(P<0.05).At baseline,there was no significant difference in serum related factors or syndrome scores between the two groups(P>0.05).After 3 months of treatment,interferon-inducible protein-10(IP-10),interleukin-6(IL-6),macrophage chemoattractant protein-1(MCP-1)and the syndrome scores of unclear vision,dry eyes,dizziness and tinnitus,dry mouth and throat in the two groups were significantly decreased,and the decrease in the observation group was greater than that in the control group with statistical difference(P<0.05).There was 1 case of rash in the observation group,and no adverse reactions occurred in the control group.Conclusion Zhixue Quyu Mingmu Tablets combined with intraocular injection of conbercept in the treatment of DME after diabetic cataract can reduce the intraocular pressure,correct the vision,reduce the symptoms and the degree of macular edema,which may be related to the regulation of related serum factors.
