摘要
目的 观察补肺固表散寒汤联合布地奈德鼻腔冲洗治疗肺气虚寒型过敏性鼻炎的疗效及对患者免疫功能、炎症反应的影响。方法 选取2019年2月—2022年9月收治的肺气虚寒型过敏性鼻炎患者200例,采用抽签分组法分为两组,单组100例。鼻腔冲洗组采取布地奈德鼻腔冲洗治疗,补肺固表组采取补肺固表散寒汤联合布地奈德鼻腔冲洗治疗。比较两组症状积分、免疫功能指标、血清相关因子及疗效的差异。结果 治疗前,两组症状积分比较,差异无统计学意义(P>0.05)。治疗后与治疗前相比,两组鼻塞、鼻痒、流涕、喷嚏、眼痒、头痛、口腔上颚疼、流泪等症状积分均明显降低,且补肺固表组治疗后较鼻腔冲洗组更低(P<0.05)。补肺固表组总有效率(93.88%,92/98)高于鼻腔冲洗组(81.63%,80/98),差异有统计学意义(P<0.05)。治疗前,两组免疫功能指标、血清相关因子比较,差异无统计学意义(P>0.05)。治疗后与治疗前相比,两组CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)及白细胞介素-10(interleukin-10,IL-10)、干扰素-γ(interferon-γ,IFN-γ)血清均明显升高,CD_(8)^(+)及血清白细胞介素-4(interleukin-4,IL-4)、白细胞介素-5(interleukin-5,IL-5)、免疫球蛋白E(immunoglobulin E,IgE)、嗜酸性粒细胞阳离子蛋白(eosinophil cationic protein, ECP)则降低,且补肺固表组治疗后CD_(8)^(+)及血清IL-4、IL-5、IgE、ECP较鼻腔冲洗组更低,CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)及IL-10、IFN-γ则更高(P<0.05)。结论 补肺固表散寒汤联合布地奈德鼻腔冲洗治疗肺气虚寒型过敏性鼻炎可有效减轻症状,改善免疫功能,抑制炎症反应,提高疗效。
Objective To observe the therapeutic effect of Bufei Gubiao Sanhan Decoction(补肺固表散寒汤)combined with budesonide nasal lavage on allergic rhinitis of lung Qi deficiency cold type and its influence on the immune function and inflammatory reaction of patients.Methods A total of 200 patients with allergic rhinitis of lung Qi deficiency cold type admitted from February 2019 to September 2022 were selected and divided into two groups by drawing lots,with 100 patients in each group.The nasal cavity flushing group was treated with budesonide nasal cavity flushing,and the lung-reinforcing and surface-strengthening group was treated with Bufei Gubiao Sanhan Decoction combined with budesonide nasal cavity flushing.The differences of symptom scores,immune function index,serum related factors and curative effect between the two groups were compared.Results Before treatment,there was no difference in symptom scores between the two groups(P>0.05).After treatment,the scores of symptoms such as nasal congestion,nasal itching,runny nose,sneezing,itchy eyes,headache,oral and maxilla pain and lacrimation in the two groups were significantly lower than those before treatment,and the scores in the lung-reinforcing and surface-strengthening group were lower than those in the nasal cavity flushing group after treatment(P<0.05).The total effective rate(93.88%,92/98)in the lung-reinforcing and surface-strengthening group was higher than that in the nasal cavity washing group(81.63%,80/98),with a significant difference(P<0.05).Before treatment,there was no difference in immune function indexes or serum related factors between the two groups(P>0.05).After treatment,compared with those before treatment,the serum levels of CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),interleukin-10(IL-10)and interferon-γ(IFN-γ)in the two groups were significantly in-creased,while CD_(8)^(+) and serum levels of interleukin-4(IL-4),interleukin-5(IL-5),immunoglobulin E(IgE)and eosinophil cationic protein(ECP)were decreased.After treatment,CD_(8)^(+) and serum levels of IL-4,IL-5,IgE and ECP in the lung-reinforcing and surface-strengthening group were lower than those in the nasal lavage group,and the levels of CD_(4)^(+) and CD_(4)^(+)/CD_(8)^(+),IL-10 and IFN-γwere higher(P<0.05).Conclusion Bufei Gubiao Sanhan Decoction combined with budesonide nasal lavage can effectively relieve symptoms,improve the immune function,inhibit the inflammatory reaction and improve the curative effect of allergic rhinitis of lung Qi deficiency cold type.
