摘要
目的 探讨利拉鲁肽联合唑来膦酸、钙剂治疗老年2型糖尿病伴骨质疏松症的效果及对血糖变异性、骨代谢的影响。方法 选取2021年1月至2023年1月我院收治的90例2型糖尿病伴骨质疏松症患者为研究对象,以随机数字表法将其分为对照组和观察组,各45例。对照组给予注射胰岛素联合唑来膦酸、钙剂治疗,观察组给予利拉鲁肽联合唑来膦酸、钙剂治疗。比较两组的治疗效果。结果 观察组的治疗总有效率高于对照组(P<0.05)。治疗后,观察组的最大血糖波动幅度(LAGE)、日内平均血糖波动幅度(MAGE)及血糖变异系数(CV)均小于对照组(P<0.05)。治疗后,观察组的Ⅰ型胶原羧基端肽β特殊序列(β-CTX)水平低于对照组,骨钙素(OC)、骨特异性碱性磷酸酶(BALP)水平高于对照组(P<0.05)。两组的不良反应总发生率无明显差异(P>0.05)。结论 利拉鲁肽联合唑来膦酸、钙剂治疗老年2型糖尿病伴骨质疏松症可提高治疗效果,促进患者血糖变异性及骨代谢的改善,且安全性较高,值得推广。
Objective To investigate the effect of liraglutide combined with zoledronic acid and calcium in the treatment of elderly type 2 diabetes mellitus complicated with osteoporosis and its influences on blood glucose variability and bone metabolism.Methods A total of 90 patients with type 2 diabetes mellitus complicated with osteoporosis admitted in our hospital from January 2021 to January 2023 were selected as the research objects and divided into control group and observation group according to the random number table method,with 45 cases in each group.The control group was treated with insulin injection combined with zoledronic acid and calcium,while the observation group was treated with liraglutide combined with zoledronic acid and calcium.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the levels of largest amplitude of glycemic excursions(LAGE),mean amplitude of glycemic excursions(MAGE)and coefficient of variation(CV)in the observation group were lower than those in the control group(P<0.05).After treatment,the level ofβ-C terminal cross-linked telopeptides of typeⅠcollagen(β-CTX)in the observation group was lower than that in the control group,and the levels of osteocalcin(OC)and bone specific alkaline phosphatase(BALP)in the observation group were higher than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Liraglutide combined with zoledronic acid and calcium in the treatment of elderly type 2 diabetes mellitus complicated with osteoporosis can improve the therapeutic effect,promote the improvement of blood glucose variability and bone metabolism,and has high safety,which is worthy of promotion.
