摘要
目的建立非奈利酮的临床合理用药评价标准,通过加权逼近理想解排序法(TOPSIS)对本院非奈利酮使用情况进行评价,为其合理应用提供依据。方法以非奈利酮的药品说明书、相关指南、专家共识、临床研究等为依据,由本院及外院医学和药学专家共同制定非奈利酮的合理用药评价标准,采用TOPSIS法合并层次分析法(AHP)对2024年2月1日至5月31日我院使用非奈利酮的279例患者的1284张门诊处方按照患者开具情况汇总后,以患者为单位进行回顾性合理用药评价。结果非奈利酮临床合理用药评价标准共建立了6个一级指标和14个二级指标。在使用非奈利酮的279例患者处方中,162例患者处方与最优方案相对接近度(C_(i))≥0.8,占比58.06%;92例患者处方为0.6≤C_(i)<0.8,占比32.98%;25例患者处方为C_(i)<0.6,占比8.96%。结论AHP赋权TOPSIS法可以用于非奈利酮临床用药合理性评价。我院非奈利酮使用情况基本合理,但仍存在不合理情况,应加强管理。
Objective To establish an evaluation criteria for the clinical rational use of finerenone and use the weighted Technique for Order Preference by Similarity to an Ideal Solution(TOPSIS)to evaluate the use of finerenone in our hospital,in order to provide an evidence for its rational application.Methods Based on the drug instructions,clinical guidelines,expert consensuses and clinical trials,medical and pharmaceutical experts from our hospital and other medical institutions jointly formulated the evaluation criteria for the rational use of finerenone,and we proposed an approach by combining TOPSIS with analytic hierarchy process(AHP),which could be used for the retrospective evaluation of the rational use of finerenone in 279 cases(1284 prescriptions)receiving outpatient service from February 1st to May 31st in 2024.Each patient with all prescriptions of finerenone was defined as a case.Results Six primary indexes and twelve secondary indexes were established in the evaluation criteria for the rational use of finerenone.Among 279 cases,relative proximity to the optimal scheme(C_(i))of the prescriptions of 162 cases(58.06%)was≥0.8;C_(i)of 92 cases(32.98%)was between 0.6~0.8(including 0.6);C_(i)of 25 cases(8.96%)was<0.6.Conclusion AHP-TOPSIS method can be used for the evaluation of the clinical rational use of finerenone.The application of finerenone is basically reasonable in our hospital,but there is still irrational use of finerenone,which should be given strengthened management.
