摘要
目的 分析多元化护理在极低出生体重早产儿视网膜病变(retinopathy of permaturity,ROP)玻璃体腔注射雷珠单抗(intravitreal ranibizumab,IVR)围手术期的应用。方法 选取2019年9月—2022年2月福建省妇幼保健院新生儿科诊断为1型ROP并行IVR的极低出生体质量患儿58例,围手术期间根据随机数字表法随机分为2组,试验组(n=29)采用多元化护理,对照组(n=29)采用传统护理,对比2组患儿一般情况、手术并发症、家属满意度及焦虑值。结果 术后2组并发症对比,对照组27.6%(8/29),试验组6.9%(2/29),试验组低于对照组,差异有统计学意义(P <0.05);术后2组复发率对比,对照组3.4%(1/29),试验组6.9%(2/29),差异无统计学意义(P> 0.05)。试验组家属满意度、疾病认知得分为(3.7±0.5)分、(6.7±0.5)分,高于对照组的(3.1±0.5)分、(3.1±1.2)分,差异均有统计学意义(P <0.001)。试验组家属的焦虑评分低于对照组[(26.9±3.9)分vs.(33.1±6.0)分],差异有统计学意义(P <0.001)。结论 多元化的护理有效提升极低出生体质量早产儿家属对疾病的认知以及满意度,缓解家属焦虑,减少治疗并发症。
Objective To evaluate diversified nursing on intravitreal ranibizumab(IVR)perioperative in very low birth weight premature infants with retinopathy of prematurity(ROP).Methods A total of 58 very low premature infants diagnosed with type 1 ROP and IVR in the department of neonatology,Fujian Maternity and Child Health Hospital from September 2019 to February 2022 were selected by randomized controlled trail design.The experimental group used diversified nursing methods during perioperative,while the control group used traditional nursing methods.The general situation,surgical complications,family satisfaction,and anxiety levels between the two groups were compared.Results The rate of postoperative complications was 27.6%(8/29)in the control group,6.9%(2/29)in the experimental group,the differences was statistically significant(P<0.05).The rate of postoperative recurrence rate of ROP in the control group was 3.4%(1/29),6.9%(2/29)in the experimental group,there was no statistically significant difference between the two groups(P>0.05).Both the satisfaction score of family members and the disease cognition score in the experimental group were(3.7±0.5)points and(6.7±0.5)points,respectively,which were higher than(3.1±0.5)points and(3.1±1.2)points in the control group,the differences were statistically significant(P<0.001).The anxiety score of the family members in the experimental group was lower than that in the control group[(26.9±3.9)points vs.(33.1±6.0)points],the differences were statistically significant(P<0.001).Conclusion Diversified nursing effectively enhances the awareness and satisfaction of families in very low birth weight premature infants with ROP,and reduces family anxiety and treatment complications.
