摘要
目的 分析富马酸替诺福韦二吡呋酯片与恩替卡韦治疗高病毒载量慢性乙型肝炎患者的临床效果及对肝功能的影响。方法 选取我院2022年2月至4月收治的80例高病毒载量慢性乙型肝炎患者作为研究对象,按照治疗方案不同将其分为替诺福韦酯组(富马酸替诺福韦二吡呋酯片治疗)和恩替卡韦组(恩替卡韦片治疗),每组40例。比较两组的治疗效果。结果 治疗4、12周,两组的慢性乙型肝炎病毒(HBV)-DNA转阴率比较,差异无统计学意义(P>0.05);治疗24、48周,替诺福韦酯组的慢性HBV-DNA转阴率显著高于恩替卡韦组,差异具有统计学意义(P<0.05)。治疗后,替诺福韦酯组的丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)、碱性磷酸酶(ALP)、总胆汁酸(TBA)水平均低于恩替卡韦组,差异具有统计学意义(P<0.05)。治疗后,替诺福韦酯组的总胆红素(TBIL)、间接胆红素(IBIL)、直接胆红素(DBIL)水平均低于恩替卡韦组,差异具有统计学意义(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。结论 在高病毒载量慢性乙型肝炎患者治疗中,富马酸替诺福韦二吡呋酯片较恩替卡韦具有更高的应用价值。
Objective To analyze the clinical effect of tenofovir disoproxil fumarate tablets and entecavir in the treatment of chronic hepatitis B patients with high viral load and its influence on liver function.Methods A total of 80 patients with chronic hepatitis B with high viral load admitted in our hospital from February to April 2022 were selected as the research objects.According to different treatment regimens,the patients were divided into tenofovir disoproxil fumarate group(treated with tenofovir disoproxil fumarate tablets)and entecavir group(treated with entecavir tablets),with 40 cases in each group.The therapeutic effects of the two groups were compared.Results After 4 and 12 weeks of treatment,there was no significant difference in the negative conversion rate of chronic hepatitis B virus(HBV)-DNA between the two groups(P>0.05);after 24 and 48 weeks of treatment,the negative conversion rate of chronic HBV-DNA in the tenofovir disoproxil fumarate group was significantly higher than that in the entecavir group,and the difference was statistically significant(P<0.05).After treatment,the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP)and total bile acids(TBA)in the tenofovir disoproxil fumarate group were lower than those in the entecavir group,and the differences were statistically significant(P<0.05).After treatment,the levels of total bilirubin(TBIL),indirect bilirubin(IBIL)and direct bilirubin(DBIL)in the tenofovir disoproxil fumarate group were lower than those in the entecavir group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion In the treatment of chronic hepatitis B patients with high viral load,tenofovir disoproxil fumarate tablets have higher application value than entecavir.
