摘要
虚热证是癌症患者临床常见的证型之一,多因恶性肿瘤日久侵袭消耗,或在现代医疗措施如放化疗等干预后形成。生石膏为清热要药,现代中医多以其大寒而将其应用局限于实热证中。生石膏之性辛、甘、微寒,质地甘润,药性纯良,在癌病之虚热证中多有应用。本研究通过追溯经典探源生石膏之药性,分析其组方应用,并介绍与总结黄挺教授使用生石膏治疗癌病之虚热证经验。此类患者临床多见乏力、口干欲饮或不欲饮、潮热汗出或夜间汗出、心烦、夜寐欠安、舌红苔少、脉细数。黄挺教授治之以清热扶正为大法,多以白虎汤及其类方为底方,以生石膏为君,配伍顾护脾胃、补益阴液之品,使邪热得清,正气得存,临证知常达变,随证灵活用药,疗效显著。其治疗经验具有较高的临床参考价值,值得深入学习和推广运用,并附医案一则以飨同道。
Asthenia pyro syndrome is one of the most common clinical types of cancer patients which is mostly caused by the Along-term invasion and consumption by malignant tumors,or formed after interventions of modern medical treatments such as radiotherapy and chemotherapy.As a heat clearing medicine,Sheng Shigao(Gypsum Fibrosum)is mostly used in excess heat syndrome due to its cold nature in in TCM in modern times.Professor HUANG Ting believes that Sheng shigao,characterized by its pungent,sweet,and slightly cold nature,as well as its moist texture,and pure and good medicinal properties,is very effective on cancer of the asthenia pyro syndrome.This article reviews the TCM classics to explore the medicinal nature of Sheng Shigao,analyzes its formula applications,and introduces and summarizes Professor HUANG Ting’s experience in using Sheng Shigao to treat asthenia pyro syndrome in cancer patients.This kind of patients often exhibit signs of asthenia pyro syndrome,clinically manifested as fatigue,dry mouth and thirst,hot flushes with sweating or nocturnal sweating,restlessness,poor appetite,sleep deficiency,red tongue with little coating,and thin rapid pulse.Professor HUANG Ting treats the disease based on the principle of clearing heat and supporting Vital-Qi,usually using the Baihu decoction(白虎汤)and its variations as the base formula,with Sheng shigao as the principal ingredient,complemented by ingredients that protect the spleen and stomach,and nourish Yin(阴)fluids.This allows the pathogenic heat to be cleared while the Vital-Qi is preserved.In clinical work,the professor can cope with different changes through known situations,uses medication flexibly according to different symptoms,achieving significant therapeutic effects.The treatment experience has high clinical reference value and is worth in-depth study and promotion.One medical case is also provided for the benefit of colleagues.
